VTN、Smad7、CEA在脑胶质瘤患者中的表达及其临床预测价值

摘要 目的 探讨玻连蛋白(VTN)、Samd同源物7(Samd7)、癌胚抗原(CEA)在脑胶质瘤患者中的表达及其临床预测价值。方法 将76例脑胶质瘤患者设为观察组,同期收治的68例无任何肿瘤性疾病的脑外伤患者设为对照组。采集两组的血清,检测VTN与CEA,另外取两组的脑组织标本以反转录-聚合酶链反应(RT-PCR)测定Samd7相对表达量,对比两组VTN、Samd7、CEA表达水平;同时依据脑胶质瘤患者的临床结局,将其分为生存组(n=56)与死亡组(n=20),比较两组VTN、Samd7、CEA表达水平;另绘制受试者工作曲线(ROC),探究VTN、Samd7、CEA三项指标单独与联合预测脑胶质瘤患者预后的价值。结果 观察组VTN水平(89.59±5.75)μg/ml、Samd7相对表达量(0.85±0.19)、CEA水平(7.88±1.49)ng/ml均高于对照组(36.78±3.67)μg/ml、(0.22±0.04)、(3.45±0.64)ng/ml,有统计学差异(P<0.05);死亡组VTN水平(78.64±7.56)μg/ml、Samd7相对表达量(0.80±0.18)、CEA水平(8.05±1.35)ng/ml均高于生存组(110.67±10.57)μg/ml、(0.94±0.21)、(9.36±1.94)ng/ml,有统计学差异(P<0.05);ROC结果显示:VTN、Samd7、CEA三项指标联合检测的曲线下面积(AUC)为0.920(95%CI:0.861~0.979),高于三项指标单独检测0.856(95%CI:0.772~0.940)、0.671(95%CI:0.528~0.814)、0.692(95%CI:0.545~0.840)。结论 脑胶质瘤患者机体内的VTN、Samd7、CEA呈异常表达,水平越高预后越差,三项指标联合检测可有效预测患者的预后,临床可大力推行。 Objective To investigate the expression of vitonectin(VTN),Samd homolog 7(Samd7) and carcinoembryonic antigen(CEA) in patients with glioma and its clinical predictive value.Methods 76 patients with brain glioma were divided into the observation group and 68 patients with brain injury without any tumor disease were divided into the control group.The serum of the 2 groups was collected to detect VTN and CEA,and the relative expression of Samd7 was measured by reverse transcription-polymerase chain reaction(RT-PCR) from the brain tissue samples of the 2 groups.The expression levels of VTN,Samd7 and CEA were compared between the 2 groups;At the same time, according to the clinical outcome of glioma patients, they were divided into survival group(n=56) and death group(n=20).The expression levels of VTN,Samd7 and CEA were compared between the 2 groups;In addition, the subject working curve(ROC) was drawn to explore the value of VTN,Samd7 and CEA in predicting the prognosis of patients with glioma.Results The VTN level in the observation group was(89.59±5.75) μg/ml, The relative expression of Samd7(0.85±0.19),and CEA level(7.88±1.49) ng/ml were higher than those of the control group(36.78±3.67) μg/ml,(0.22±0.04),(3.45±0.64) ng/ml, with statistical difference(P<0.05);VTN level in death group(78.64±7.56) μg/ml, The relative expression of Samd7(0.80±0.18),and CEA level(8.05±1.35) ng/ml were higher than those in the survival group(110.67±10.57) μg/ml,(0.94±0.21),(9.36±1.94) ng/ml, with statistical difference(P<0.05);The ROC results showed that the area under the curve(AUC) of the combined detection of VTN,Samd7 and CEA was 0.920(95% CI:0.861~0.979),which was higher than that of the independent detection of the 3 indicators: 0.856(95% CI:0.772~0.940),0.671(95% CI:0.528~0.814),0.692(95% CI:0.545~0.840).Conclusion The VTN,Samd7 and CEA in the body of glioma patients are abnormally expressed, and the higher the level, the worse the prognosis.The combined detection of the three indicators can effectively predict the prognosis of patients, which can be vigorously promoted in clinical practice.
