宫颈癌患者血清MMP-1、MMP-2水平与预后的关系 被引量:4

Relationship between serum MMP-1, MMP-2 levels and prognosis in patients with cervical cancer

摘要 目的探究宫颈癌患者血清基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-2(MMP-2)水平与预后的关系。方法选取本院2017年1月至2019年10月收治的69例宫颈癌患者(癌症组)、75例同年龄层来我院体检的健康女性(对照组)和72例宫颈上皮内瘤样病变(CIN)患者(CIN组)为研究对象。比较三组研究对象的血清MMP-1、MMP-2水平;分析宫颈癌患者血清MMP-1、MMP-2水平与临床病理的关系;采用Kaplan-Meier生存曲线分析血清MMP-1、MMP-2水平与宫颈癌患者预后的关系;通过Cox回归模型分析宫颈癌患者预后的影响因素;采用受试者工作特征(ROC)曲线评估血清MMP-1、MMP-2水平及二者联合对宫颈癌的诊断价值。结果癌症组的血清MMP-1、MMP-2水平高于CIN组和对照组(P<0.05)。MMP-1、MMP-2高水平组的3年总生存率低于低水平组(P<0.05)。Cox回归分析显示,国际妇产科联盟(FIGO)分期、淋巴结转移、MMP-1及MMP-2水平是影响宫颈癌患者预后的危险因素(P<0.05)。MMP-1、MMP-2联合诊断宫颈癌的曲线下面积(AUC)明显大于单一MMP-1或MMP-2诊断(P<0.05)。结论血清MMP-1、MMP-2水平是影响宫颈癌患者的生存时间的重要因素;MMP-1、MMP-2联合检测对宫颈癌具有良好的诊断效能,对临床早期筛查发现宫颈癌有重要价值。 Objective To investigate the relationship between serum matrix metalloproteinase-1(MMP-1),matrix metalloproteinase-2(MMP-2)levels and prognosis in patients with cervical cancer.Methods Sixty-nine patients with cervical cancer admitted in our hospital from January 2017 to October 2019(cancer group),75 healthy women of the same age group who came to our hospital for physical examination(control group)and 72 patients with cervical intraepithelial neoplasia(CIN)(CIN group)were selected as the research objects.The levels of serum MMP-1 and MMP-2 were compared among the three groups;the relationship between serum MMP-1 and MMP-2 levels and clinicopathology in patients with cervical cancer were analyzed;Kaplan-Meier survival curve was used to analyze the relationship between serum MMP-1 and MMP-2 levels and prognosis of patients with cervical cancer;the factors influencing prognosis of patients with cervical cancer were analyzed by Cox regression model;receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of serum MMP-1,MMP-2 and their combination for cervical cancer.Results The levels of serum MMP-1 and MMP-2 in the cancer group were higher than those in the CIN group and the control group(P<0.05).The 3-year overall survival rate of high level MMP-1 and MMP-2 groups were lower than those of low level group(P<0.05).The Cox regression analysis showed that International Federation of Gynecology and Obstetrics(FIGO)stage,lymph node metastasis,MMP-1 and MMP-2 levels were risk factors for the prognosis of cervical cancer patients(P<0.05).The area under curve(AUC)of MMP-1 and MMP-2 in the combined diagnosis of cervical cancer was significantly higher than that of single MMP-1 or MMP-2(P<0.05).Conclusion The levels of serum MMP-1 and MMP-2 are important factors affecting the survival time of patients with cervical cancer;the combined detection of MMP-1 and MMP-2 has good diagnostic efficacy for cervical cancer,and has important value for clinical early screening and detection of cervical cancer.
