经导管选择性脑动脉内介入溶栓术联合鼠神经生长因子治疗缺血性脑梗死的效果及对颅内血流动力学、神经因子水平的影响 被引量:2

摘要 目的探讨经导管选择性脑动脉内介入溶栓术联合鼠神经生长因子治疗缺血性脑梗死的效果及对颅内血流动力学、神经因子水平的影响。方法选取2019年6月至2021年6月收治的90例缺血性脑梗死患者为研究对象,根据患者病情与治疗意愿将其分为对照组与观察组,各45例。对照组采用常规治疗,观察组采用经导管选择性脑动脉内介入溶栓术+鼠神经生长因子治疗。比较两组的治疗效果。结果治疗后,观察组的美国国立卫生研究院卒中量表(NIHSS)评分低于对照组(P<0.05);观察组的治疗总有效率为95.56%,高于对照组的75.56%(P<0.05)。治疗后,观察组的大脑中动脉(MCA)、大脑前动脉(ACA)平均血流速度高于对照组(P<0.05)。治疗后,观察组的脑源性神经营养因子(BDNF)、神经生长因子(NGF)水平高于对照组,胶质纤维酸性蛋白(GFAP)水平低于对照组(P<0.05)。治疗后,观察组的世界卫生组织生活质量简表(WHOQOL-BREF)各领域评分均高于对照组(P<0.05)。结论经导管选择性脑动脉内介入溶栓术联合鼠神经生长因子治疗缺血性脑梗死的效果满意,能改善患者的颅内血流动力学,调节神经因子水平,改善生活质量,值得推广。 Objective To explore the effect of transcatheter selective intra-arterial interventional thrombolysis combined with mouse nerve growth factor in the treatment of ischemic cerebral infarction and its influences on intracranial hemodynamics and nerve factors levels. Methods Ninety patients with ischemic cerebral infarction admitted from June 2019to June 2021 were selected as the research objects. According to the patients’ condition and treatment intention, they were divided into control group and observation group, with 45 cases in each group. The control group was treated with conventional treatment, and the observation group was treated with transcatheter selective intra-arterial interventional thrombolysis + mouse nerve growth factor. The therapeutic effects of the two groups were compared. Results After treatment, the National Institutes of Health Stroke Scale(NIHSS) score in the observation group was lower than that in the control group(P<0.05);the total effective rate of treatment in the observation group was 95.56%, which was higher than75.56% in control group(P <0.05). After treatment, the mean blood flow velocity of middle cerebral artery(MCA) and anterior cerebral artery(ACA) in the observation group were higher than those in the control group(P <0.05). After treatment, the levels of brain-derived neurotrophic factor(BDNF) and nerve growth factor(NGF) in the observation group were higher than those in the control group, while the level of glial fibrillary acidic protein(GFAP) was lower than that in the control group(P<0.05). After treatment, WHO Quality of Life-BREF(WHOQOL-BREF) scores of all fields in the observation group were higher than those in the control group(P<0.05). Conclusion Transcatheter selective intra-arterial interventional thrombolysis combined with mouse nerve growth factor in the treatment of ischemic cerebral infarction has a satisfactory effect, it can improve intracranial hemodynamics, regulate the nerve factors levels and improve the quality of life of patients, which is worth promoting.
