复方血栓通联合玻璃体腔注射康柏西普治疗渗出型老年性黄斑变性的效果及对血管内皮功能、中心凹旁表层视网膜血管密度的影响

Effect of compound Xueshuantong combined with intravitreal injection of conbercept in the treatment of exudative age-related macular degeneration and its influences on vascular endothelial function and retinal vascular density of the surface layer near the fovea

目的探讨复方血栓通联合玻璃体腔注射康柏西普治疗渗出型老年性黄斑变性的效果。方法选取2017年3月至2021年3月我院收治的渗出型老年性黄斑变性患者48例,以随机数字表法将其分为对照组和观察组,各24例。对照组采用玻璃体腔注射康柏西普治疗,观察组采用复方血栓通联合玻璃体腔注射康柏西普治疗,比较两组的治疗效果。结果治疗后,观察组的最佳矫正视力(BCVA)高于对照组,眼压、黄斑中心凹视网膜厚度(CMT)低于对照组(P<0.05)。治疗后,观察组的血管内皮生长因子(VEGF)、血小板衍生生长因子(PDGF)水平低于对照组,内皮抑素(ES)水平高于对照组(P<0.05)。治疗后,观察组的中心凹旁表层视网膜血管密度、血流灌注水平均高于对照组(P<0.05)。治疗后,观察组的收缩期峰值流速(PSV)、舒张末期流速(EDV)高于对照组,阻力指数(RI)低于对照组(P<0.05)。结论复方血栓通联合玻璃体腔注射康柏西普治疗渗出型老年性黄斑变性的效果显著,值得推广。

Objective To explore the effect of compound Xueshuantong combined with intravitreal injection of conbercept in the treatment of exudative age-related macular degeneration. Methods Forty-eight patients with exudative age-related macular degeneration admitted in our hospital from March 2017 to March 2021 were selected and divided into control group and observation group by random number table method, with 24 cases in each group. The control group was treated with intravitreal injection of conbercept, and the observation group was treated with compound Xueshuantong combined with intravitreal injection of conbercept. The therapeutic effects of the two groups were compared. Results After treatment, the best corrected visual acuity(BCVA) in the observation group was higher than that in the control group, while the intraocular pressure and central macular thickness(CMT) were lower than those in the control group(P<0.05). After treatment, the levels of vascular endothelial growth factor(VEGF) and platelet-derived growth factor(PDGF) in the observation group were lower than those in the control group, while the level of endostatin(ES) was higher than that in the control group(P<0.05).After treatment, the retinal vascular density of the surface layer near the fovea and blood perfusion level in the observation group were higher than those in the control group(P<0.05). After treatment, peak systolic velocity(PSV) and end diastolic velocity(EDV) in the observation group were higher than those in the control group, and resistance index(RI) was lower than that in the control group(P<0.05). Conclusion Compound Xueshuantong combined with intravitreal injection of conbercept in the treatment of exudative age-related macular degeneration has a remarkable effect, which is worth popularizing.
