分阶段体位及呼吸结合骨盆摇摆对高龄产妇分娩自控感、分娩信心及自我效能感的影响

Effects of staged position and breathing combined with pelvic swing on childbirth self-control, childbirth confidence and self-efficacy of elderly parturient women

目的探讨分阶段体位及呼吸结合骨盆摇摆在高龄产妇中的应用效果。方法选择我院2019年5月至2021年3月收治的144例高龄产妇为研究对象,采用随机数字表法将其分为对照组与观察组,各72例。对照组接受常规分娩护理,观察组实施分阶段体位及呼吸结合骨盆摇摆。比较两组的干预效果。结果观察组的简化版McGill疼痛问卷(SF-MPQ)各分量评分均低于对照组,分娩控制量表(LAS)评分高于对照组,第一、二产程时间短于对照组(P<0.05)。干预后,观察组的一般自我效能感量表(GSES)及分娩信心评估量表评分均高于对照组(P<0.05)。分娩后2 h,观察组的肾上腺素(E)、皮质醇(Cor)水平低于对照组(P<0.05)。结论分阶段体位及呼吸结合骨盆摇摆能减轻高龄产妇分娩疼痛,缩短产程时间,提高分娩自控感、分娩信心及自我效能感,也能减轻机体应激反应,值得推广。

Objective To explore the application effect of staged position and breathing combined with pelvic swing in elderly parturient women. Methods A total of 144 elderly parturient women admitted in our hospital from May 2019 to March 2021 were selected as the research objects and divided into control group and observation group by random number table method, with 72 cases in each group. The control group received routine childbirth nursing, and the observation group implemented staged position and breathing combined with pelvic swing. The intervention effects of the two groups were compared. Results The scores of each component of the Simplified McGill Pain Questionnaire(SF-MPQ) in the observation group were lower than those in the control group, the score of Labor Agentry Scale(LAS) was higher than that in the control group, and the first and second stage of labor were shorter than those in the control group(P <0.05). After intervention, the scores of General Self-Efficacy Scale(GSES) and childbirth confidence assessment scale in the observation group were higher than those in the control group(P<0.05). At 2 h after childbirth, the levels of epinephrine(E)and cortisol(Cor) in the observation group were lower than those in the control group(P<0.05). Conclusion The staged position and breathing combined with pelvic swing can reduce the labor pain of elderly parturient women, shorten the duration of labor, improve the sense of childbirth self-control, childbirth confidence and self-efficacy, and also reduce the body’s stress response, which is worthy of promotion.
