FIS1 mRNA、HPV E6/E7 mRNA表达水平与宫颈病变严重程度的关系研究

The relationship between the expression levels of FIS1 mRNA and HPV E6/E7 mRNA and the severity of cervical lesions

目的 研究线粒体分裂蛋白1(FIS1)mRNA、人乳头瘤病毒(HPV)E6/E7 mRNA表达水平与宫颈病变严重程度的关系。方法 收集张家口市妇幼保健院2021年1月-2021年7月行宫颈癌筛查的5666例标本,经宫颈液基薄层细胞学检查(TCT)诊断285例不典型鳞状细胞(ASC)、183例鳞状上皮内低度病变(LSIL)、98例鳞状上皮内高度病变(HSIL)、10例宫颈癌;经病理活检诊断356例未明确诊断意义的不典型鳞状上皮细胞(ASCUS)、218例宫颈上皮内瘤样病变(CIN)Ⅰ级、42例CINⅡ级、24例CINⅢ级、15例宫颈鳞状细胞癌。分别参考TCT、病理活检诊断结果分析不同宫颈病变程度的FIS1 mRNA、HPV E6/E7 mRNA阳性检出率与表达量。通过Pearson分析FIS1 mRNA与HPV E6/E7 mRNA阳性表达与宫颈病变严重程度的相关性。结果 以TCT不同诊断级别为参考,FIS1 mRNA、HPV E6/E7 mRNA表达对LSIL、HSIL、宫颈鳞状细胞癌的阳性检出率高于ASC(P<0.05)。FIS1 mRNA、HPV E6/E7 mRNA表达对宫颈鳞状细胞癌的阳性检出率高于LSIL(P<0.05)。FIS1 mRNA拷贝数:宫颈癌、HSIL<LSIL<ASC(P<0.05);HPV E6/E7 mRNA拷贝数:宫颈癌>HSIL>LSIL>ASC(P<0.05)。以病理活检结果为诊断标准,FIS1 mRNA、HPV E6/E7 mRNA表达对CINⅡ级、CINⅢ级、宫颈鳞状细胞癌的阳性检出率高于ASCUS、CINⅠ级(P<0.05);FIS1 mRNA拷贝数:宫颈鳞状细胞癌、CINⅢ级<CINⅡ级<CINⅠ级<ASCUS(P<0.05);HPV E6/E7 mRNA拷贝数:宫颈鳞状细胞癌>CINⅢ级>CINⅡ级>CINⅠ级、ASCUS(P<0.05)。Pearson相关性分析结果显示,宫颈病变严重程度与FIS1 mRNA表达量呈显著负相关性(r=-0.881,P<0.05),与HPV E6/E7 mRNA表达量呈显著正相关性(r=0.903,P<0.05)。结论 FIS1 mRNA、HPV E6/E7 mRNA表达水平与宫颈病变程度存在显著相关性,宫颈病变程度越严重,FIS1mRNA表达量呈降低趋势,而HPV E6/E7 mRNA表达量呈上升趋势。

Objective To study the relationship between the expression levels of mitochondrial fission protein 1(FIS1) mRNA,human papillomavirus(HPV) E6/E7 mRNA and the severity of cervical lesions.Methods A total of 5666 cases of cervical cancer screening were collected in Zhangjiakou Maternal and Child Health Hospital from January 2021 to July 2021,and 285 cases of atypical squamous cells(ASC),183 cases of atypical squamous cells(ASC) and 183 cases of Cases of low-grade squamous intraepithelial lesions(LSIL),98 cases of high-grade squamous intraepithelial lesions(HSIL),and 10 cases of cervical cancer.Pathological biopsy diagnosed 356 cases of atypical squamous cells of undetetemined sign(ASCUS),218 cases of cervical intraepithelial neoplasia(CIN) grade Ⅰ,42 cases of CIN grade Ⅱ,24 cases of CIN grade Ⅲ,and 15 cases of cervical Squamous cell carcinoma.The positive detection rate and expression of FIS1 mRNA and HPV E6/E7 mRNA in different cervical lesions were analyzed with reference to TCT and pathological biopsy diagnosis results.The correlation between the positive expression of FIS1 mRNA and HPV E6/E7 mRNA and the severity of cervical lesions was analyzed by Pearson.Results Taking the different diagnostic grades of TCT as a reference, the positive detection rate of FIS1 mRNA and HPV E6/E7 mRNA expression in LSIL,HSIL and cervical squamous cell carcinoma was higher than that in ASC(P<0.05).The positive detection rate of FIS1 mRNA and HPV E6/E7 mRNA expression in cervical squamous cell carcinoma was higher than that in LSIL(P<0.05).FIS1 mRNA copy number: cervical cancer, HSILHSIL>LSIL>ASC(P<0.05).Taking pathological biopsy results as the diagnostic criteria, the positive detection rates of FIS1 mRNA and HPV E6/E7 mRNA in CIN Ⅱ,CIN Ⅲ and cervical squamous cell carcinoma were higher than those in ASCUS and CIN I(P<0.05);FIS1 mRNA Copy number: cervical squamous cell carcinoma, CIN grade ⅢCIN grade Ⅲ>CIN grade Ⅱ>CIN Ⅰ grade, ASCUS(P<0.05).The results of Pearson correlation analysis showed that the severity of cervical lesions was significantly negatively correlated with FIS1 mRNA expression(r=-0.881,P<0.05),and significantly positively correlated with HPV E6/E7 mRNA expression(r=0.903,P<0.05).Conclusion There was a significant correlation between the expression levels of FIS1 mRNA and HPV E6/E7 mRNA and the degree of cervical lesions.The more severe the cervical lesions were, the lower the expression of FIS1 mRNA,and the higher the expression of HPV E6/E7 mRNA.
