5例HIV阴性的浆母细胞淋巴瘤临床病理分析

Clinicopathologic analysis of 5 cases of HIV-negative plasma cell lymphoma

目的 探讨浆母细胞淋巴瘤(PBL)的临床病理特征及诊断、鉴别诊断。方法 对5例浆母细胞淋巴瘤的临床病理特征及免疫表型进行分析,并查阅相关文献及资料,对病理诊断及鉴别诊断进行总结。结果 5例患者HIV均为阴性,均发生于淋巴结外,均表现免疫母细胞的形态学特征并表达浆细胞的免疫标记。Ki-67增值指数均较高(70%~95%,平均83%)。结论 浆母细胞淋巴瘤是一种罕见的淋巴瘤,形态学上表现出免疫母细胞的形态特征,但表达浆细胞的免疫标记,临床进展迅速,预后差,治疗及诊断仍然是一个难题,更有效的诊断水平及治疗方案有待进一步研究。

Objective To investigate the clinicopathological features, diagnosis and differential diagnosis of plasma blast cell lymphoma(PBL). Methods The clinicopathological features and immunophenotypes of 5 cases of plasmacytoblastic lymphoma were analyzed, and the pathological diagnosis and differential diagnosis were summarized by referring to relevant literature and data. Results All 5 patients were HIV negative. All 5 cases occurred outside lymph nodes. All patients showed morphological characteristics of immunoblast and plasma cell immunophenotype. Ki-67 value-added index was higher(70%-95%, average 83%). Conclusion Plasmacytoblast lymphoma is a rare type of lymphoma, similar to immunoblast in morphology, but with plasma cell immunophenotype, rapid clinical progress, poor prognosis. Treatment and diagnosis are still a challenge, and more effective regimens need to be further studied.
