人体测量参数对宁夏农村地区血脂异常发生风险的评估 被引量:1

摘要 目的探究宁夏农村地区居民的血脂水平与人体测量参数之间的关系,并比较五种人体测量参数对血脂异常发生风险的预测能力。方法按照多阶段抽样的方法选取宁夏农村地区居民(n=14360)进行人体测量特征、血脂水平评估。使用多元线性回归评估人体测量参数与血脂水平之间的关联;根据血脂指标结果,按照是否血脂异常分成血脂正常组与血脂异常组。使用Logistic回归分析人体测量参数作为血脂异常危险因素的危险性;使用受试者工作特征曲线比较各人体测量参数对血脂异常发生的预测能力。结果各人体测量参数对血脂指标方差的解释程度较低,但均具有统计学意义(P均<0.05);腰臀比是血脂异常患病风险的最强危险因素(OR=1.402,P<0.001),其中腰臀比是TC、HDL-C和LDL-C异常患病风险的最强危险因素(OR=1.351、1.309、1.436,P均<0.05);腰高比是TG水平的最强危险因素(OR=1.531,P<0.001);腰围对男性血脂异常的预测能力较强(AUC=0.642),腰高比对女性血脂异常的预测能力较强(AUC=0.606)。结论体质量指数、腰围、腰高比、腰臀比、臀围与血脂异常密切相关,腰围能更好地预测男性血脂异常发生风险,而腰高比能更好地预测女性血脂异常发生风险。 Objective To explore the relationship between blood lipid level and anthropometric parameters in rural residents of Ningxia,and compare the predictive ability of five anthropometric parameters on the risk of dyslipidemia.Methods According to the method of multi-stage sampling,the rural residents(n=14360)in Ningxia were selected for anthropometric characteristics and blood lipid level assessment.Multivariate linear regression was used to evaluate the correlation between anthropometric parameters and blood lipid levels;According to the results of blood lipid indicators,they were divided into ortholiposis group and dyslipidemic group according to whether the blood lipid was abnormal or not.The risk of anthropometric parameters as risk factors of blood lipid abnormality was compared using Logistic regression;The prediction ability of anthropometric parameters on the occurrence of dyslipidemia was compared using the subject work characteristic curve.Results The explanation degree of each anthropometric parameter to the variance of blood lipid index was low,but all had statistical significance(P all<0.05);The waist hip ratio was the strongest risk factor for the prevalence of dyslipidemia(OR=1.402,P<0.001),among which,the waist hip ratio was the strongest risk factor for the prevalence of TC,HDL-C and LDL-C abnormalities(OR=1.351,1.309,1.436,P all<0.05);The waist height ratio was the strongest risk factor for TG level(OR=1.531,P<0.001);Waist circumference has a higher predictive ability for male dyslipidemia(AUC=0.642),and waist height ratio has a higher predictive ability for female dyslipidemia(AUC=0.606).Conclusion Body mass index,waist circumference,waist height ratio,waist hip ratio,hip are closely related to dyslipidemia.Waist circumference can better predict the risk of male dyslipidemia,while waist height ratio can better predict the risk of female dyslipidemia.
