老年髋关节感染的影响因素及RANKL与DUSP5和miR-205对其预测价值

Influencing factors of hip infection in the elderly and the predictive value of RANKL and DUSP5 and miR-205

目的分析老年髋关节感染的影响因素及人核因子-κB受体活化因子配体(RANKL)、双特异性磷酸酶5(DUSP5)、微小RNA-205(miR-205)对其预测价值。方法选取2019年2月-2022年2月于同济大学附属第十人民医院进行治疗髋关节及致髋关节出现病变的患者103例,其中出现髋关节感染的患者53例,其余50例为未感染患者,同期选取50名进行健康体检者为本研究的对照组。采用双抗体夹心酶联免疫吸附法检测RANKL及炎症指标,采用实时逆转录聚合酶链反应(RT-PCR)法测定DUSP5、miR-205相对表达量;采用Logistic回归分析归纳老年髋关节感染的影响因素,采用受试者工作特征(ROC)曲线分析RANKL、DUSP5、miR-205预测髋关节感染的预测价值。结果与未感染组对比,感染组RANKL、DUSP5、miR-205表达量升高,Logistic回归分析RANKL、DUSP5、miR-205、CRP、IL-6和PCT是老年患者髋关节感染的影响因素(P<0.05);RANKL、DUSP5、miR-205预测老年患者髋关节感染的曲线下面积(AUC)分别为0.917、0.870、0.853。结论髋关节感染患者中的RANKL、DUSP5、miR-205均显著升高,高水平RANKL、DUSP5、miR-205对老年髋关节感染有一定的预测价值。

OBJECTIVE To analyze the influencing factors of hip infection in the elderly and the predictive value of human nuclear factor-κB receptor activator ligand(RANKL),dual-specificity phosphatase 5(DUSP5)and microRNA-205(miR-205)on its prognosis.METHODS Totally 103 patients underwent hip joint or hip joint lesions treatment who were hospitalized in Tenth People′s Hospital Affiliated to Tongji University from Feb.2019 to Feb.2022 were selected,including 53 patients with hip joint infection,and the remaining 50 patients were uninfected patients.Fifty patients who underwent physical examination during the same period were selected as the control group for this study.Double antibody sandwich enzyme-linked immunosorbent assay was used to detect RANKL and inflammatory indexes,and real-time reverse transcription polymerase chain reaction(RT-PCR)method was used to determine the relative expression of DUSP5 and miR-205.Logistic regression analysis was used to summarize the influencing factors of hip joint infection in the elderly,and the receiver operating curve(ROC)was used to analyze the predictive value of RANKL,DUSP5,and miR-205 in predicting the occurrence of hip infection.RESULTS Compared with the uninfected group,the expressions of RANKL,DUSP5,and miR-205 in the infected group were increased.Logistic regression analysis showed that the expressions of RANKL,DUSP5,miR-205,CRp,IL-6 and PCT were the influencing factors of hip joint infection in the elderly patients(P<0.05).The area under the curve(AUC)of of RANKL,DUSP5,and miR-205 for predicting hip infection in elderly patients were 0.917,0.870,and 0.853,respectively.CONCLUSION The levels of RANKL,DUSP5,and miR-205 in patients with hip joint infection were significantly increased,and high levels of RANKL,DUSP5,and miR-205 had certain predictive value for hip joint infection in elderly patients.
