艾司氯胺酮复合丙泊酚对腹腔镜下小儿腹股沟疝疝囊结扎术麻醉质量的影响

Effect of Esketamine Combined with Propofol on Anesthesia Quality in Laparoscopic Inguinal Hernia Sac Ligation in Children

目的:探究艾司氯胺酮复合丙泊酚对腹腔镜下小儿腹股沟疝疝囊结扎术麻醉质量的影响。方法:选择2021年4月-2022年10月江苏省新沂市人民医院收治的83例腹股沟疝行疝囊结扎术患儿作为研究对象,将患儿按照入院先后顺序进行编号,采用随机数字表法分为对照组和观察组。对照组给予丙泊酚复合常规麻醉,观察组给予艾司氯胺酮复合丙泊酚麻醉。比较两组麻醉相关指标、血流动力学指标、不良反应发生情况。结果:麻醉后,观察组苏醒时间、麻醉恢复室滞留时间、清醒时间均短于对照组,差异有统计学意义(P<0.05)。麻醉前,两组平均动脉压(MAP)、心率(HR)、血氧饱和度(SpO_(2))比较,差异无统计学意义(P>0.05);麻醉后,观察组MAP、HR低于对照组,差异有统计学意义(P<0.05);两组SpO_(2)比较,差异无统计学意义(P>0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.041)。结论:艾司氯胺酮复合丙泊酚麻醉应用于小儿腹股沟疝疝囊结扎术中,能够稳定患儿血流动力学,提升麻醉质量,降低不良反应发生率,缩短患儿术后苏醒时间。

Objective:To explore the effect of esketamine combined with propofol on the quality of anesthesia in laparoscopic inguinal hernia sac ligation in children.Methods:A total of 83 children with inguinal hernia who underwent hernia sac ligation in Xinyi People's Hospital of Jiangsu Province from April 2021 to October 2022 were selected as the study subjects.The children were numbered according to the order of admission and divided into control group and observation group according to random number table method.The control group received conventional anesthesia with propofol and the observation group received combined anesthesia of esketamine and propofol.The anesthesia related indexes,hemodynamic indexes and the occurrence of adverse reactions were compared between the two groups.Results:After anesthesia,the recovery time,retention time of anesthesia recovery room,and waking time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).Before anesthesia,there was no significant difference in mean arterial pressure(MAP),heart rate(HR),and blood oxygen saturation(SpO_(2))between the two groups(P>0.05).After anesthesia,the MAP and HR in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in SpO_(2) between the two groups(P>0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.041).Conclusion:The application of combined general anesthesia with low-dose ketamine and propofol in the ligation of inguinal hernia sac in children can stabilize hemodynamics,improve anesthesia quality,reduce the incidence of adverse reactions,and shorten the time of postoperative recovery of children.
