基于急性胃肠功能损伤分级标准的护理模式在急性胰腺炎患者中的应用效果

Application Effect of Nursing Mode Based on Grading Standard of Acute Gastrointestinal Injury in Patients with Acute Pancreatitis

目的:探讨基于急性胃肠功能损伤(AGI)分级标准的护理模式对急性胰腺炎患者营养状态及生活质量的影响。方法:选取2020年8月-2022年8月南京市江宁医院消化内科收治的96例急性胰腺炎患者作为研究对象,以计算机随机分为研究组与对照组,各48例。对照组实施常规护理,研究组以AGI分级标准为依据实施分级护理。比较两组肠屏障功能、营养状态、生活质量。结果:入院时,两组肠脂肪酸结合蛋白、内毒素水平比较,差异无统计学意义(P>0.05);干预后,研究组肠脂肪酸结合蛋白、内毒素水平均低于对照组,差异有统计学意义(P<0.05)。入院时,两组前白蛋白、血清白蛋白、体脂率比较,差异无统计学意义(P>0.05);干预后,研究组前白蛋白、血清白蛋白、体脂率均高于对照组,差异有统计学意义(P<0.05)。入院时,两组社交活动、心理、生理功能、自觉症状评分比较,差异无统计学意义(P>0.05);干预后,研究组社交活动、心理、生理功能、自觉症状评分均高于对照组,差异有统计学意义(P<0.05)。结论:对急性胰腺炎患者实施基于AGI分级标准的护理干预,能有效改善患者的肠屏障功能和营养状态,提升其生活质量。

Objective:To explore the influence of the nursing mode based on the grading standard of acute gastrointestinal injury(AGI)on the nutritional status and quality of life of patients with acute pancreatitis.Methods:A total of 96 patients with acute pancreatitis admitted to the Gastroenterology Department of Nanjing Jiangning Hospital from August 2020 to August 2022 were selected as the study subjects,and they were randomly divided into study group and control group by computer,with 48 cases in each group.The control group implemented routine nursing,and the study group implemented graded nursing based on the AGI grading standard.The intestinal barrier function,nutritional status and quality of life in the two groups were compared.Results:At admission,there was no significant difference in intestinal fatty acid-binding protein and endotoxin levels between the two groups(P>0.05);After the intervention,the levels of intestinal fatty acid-binding protein and endotoxin in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).At admission,there was no significant difference in the prealbumin,serum albumin and body fat rates between the two groups(P>0.05).After the intervention,the prealbumin,serum albumin and body fat rates in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).At admission,there was no significant difference in the scores of social activities,psychological,physiological function,and subjective symptoms between the two groups(P>0.05);After the intervention,the scores of social activities,psychological,physiological function and subjective symptoms in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:For patients with acute pancreatitis,nursing intervention based on AGI grading standard can effectively improve the intestinal barrier function and nutritional status of patients,and improve the quality of life.
