加味清脑方治疗气滞血瘀型卒中后抑郁临床研究

Clinical Study on the Treatment of Post-Stroke Depression of Qi Stagnation and Blood Stasis Type with Modified Qingnao Formula

目的:观察加味清脑方治疗气滞血瘀型卒中后抑郁的临床疗效。方法:将76例气滞血瘀型卒中后抑郁患者按照随机数字表法分为对照组和治疗组,每组各38例。两组均给予原发病治疗,对照组给予氟哌噻吨美利曲辛片治疗,治疗组在对照组治疗的基础上口服加味清脑方。比较两组患者的临床疗效及治疗前后汉密尔顿焦虑量表(hamilton depression scale,HAMD)评分、美国国立卫生院神经功能缺损评分(national institutes of health neurological deficit score,NIHSS)、Barthel指数量表(barthel index,BI)评分变化情况。结果:两组患者治疗后HAMD评分低于本组治疗前,且治疗后治疗组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后NIHSS评分低于本组治疗前,且治疗后治疗组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后BI评分高于本组治疗前,且治疗后治疗组高于对照组,差异具有统计学意义(P<0.05)。对照组有效率为26.31%,治疗组有效率为68.42%,两组患者有效率比较,差异具有统计学意义(P<0.05)。结论:加味清脑方治疗气滞血瘀型卒中后抑郁,可改善患者抑郁状态及神经功能缺损情况,提高患者的日常生活能力。

Objective:To observe the clinical efficacy of Supplemented Brain-Clearing Decoction on post-stroke depression(PSD) of qi stagnation and blood stasis type.Methods:A total of 76 patients with PSD of qi stagnation and blood stasis type were randomly divided into the control group and the treatment group,with 38 cases in each group.Both groups were first treated with conventional Western medicine,then the control group was additionally treated with Dropethidol Melitracen Tablets,while the treatment group was additionally treated with Supplemented Brain-Clearing Decoction on the basis of the treatment of the control group.The clinical efficacy,the changes of the scores of Hamilton anxiety scale(HAMD),National Institutes of Health neurological defect score(NIHSS) and Barthel index(BI) of the two groups before and after treatment were compared.Results:After treatment,the HAMD score of the two groups was lower than that of the same group before treatment,and the treatment group′s was lower than that of the control group,and the difference was statistically significant(P<0.05);The NIHSS score of the two groups was lower than that of the same group before treatment,and the treatment group′s was lower than that of the control group,and the difference was statistically significant(P<0.05);The BI score of the two groups was higher than that of the same group before treatment,and the treatment group′s was higher than that of the control group,and the difference was statistically significant(P<0.05).The effective rate was 26.31% in the control group and 68.42% in the treatment group.The difference between the two groups was statistically significant(P<0.05).Conclusion:Supplemented Brain-Clearing Decoction can improve the depression and neurological function defects and enhance the ability of daily life of patients with PSD of qi stagnation and blood stasis type.
