中药刮痧膏剂对脑卒中后肩手综合征患者运动功能的影响

The Influence of Chinese Medicinal Plaster for Scraping on Motor Function in Patients with Shoulder-Hand Syndrome After Stroke

目的:观察以中药刮痧膏剂作为介质进行刮痧结合综合康复疗法对脑卒中后肩手综合征患者的影响。方法:选取2020年10月至2021年12月就诊于河南省中医院、河南中医药大学第三附属医院、河南省中医药研究院附属医院康复科的脑卒中后肩手综合征患者80例,按照随机数字表法分为对照组和治疗组,每组40例。对照组给予对症治疗、康复训练、刮痧治疗,治疗组给予对症治疗、康复训练、刮痧治疗,刮痧时采用中药刮痧膏剂作为介质进行刮拭,其他治疗及刮痧操作方法同对照组。观察两组患者治疗前后中医证候评分、改良Fulg-Meyer量表(Fugl-Meyer assessment,FMA)评分、疼痛视觉模拟评分(visual analogue score,VAS)、日常生活活动能力量表(activity of daily living scale,ADL)评分变化情况及临床疗效。结果:治疗组治疗后中医证候评分、VAS评分低于对照组,FMA评分、ADL评分高于对照组,差异均有统计学意义(P<0.05);治疗组有效率为97.50%,对照组有效率为82.50%,治疗组有效率高于对照组,差异有统计学意义(P<0.05)。结论:运用中药刮痧膏剂作为介质进行刮痧结合康复疗法治疗脑卒中后肩手综合征能有效减轻患者肩部疼痛,改善临床症状,提高患者日常生活活动能力。

Objective:To observe the influence of Chinese medicinal plaster for scraping as medium scraping combined with comprehensive rehabilitation therapy on patients with shoulder-hand syndrome after stroke.Methods:A total of 80 patients with shoulder-hand syndrome after stroke treated in the Rehabilitation Department of Henan Province Hospital of TCM,Third Affiliated Hospital of Henan University of Traditional Chinese Medicine,Rehabilitation Department of Affiliated Hospital of Henan Academy of Chinese Medicine from October 2020 to December 2021 were selected and divided into the control group and the treatment group according to random number table,with 40 cases in each group.The control group was given symptomatic treatment,rehabilitation training and scraping treatment,while the treatment group was also given symptomatic treatment,rehabilitation training and scraping treatment,but during scraping,Chinese medicinal plaster for scraping was used as the medium for scraping.Other treatment and scraping operation methods were the same as those of the control group.Then the changes of TCM syndrome score,Fugl-Meyer assessment(FMA) score,visual analog score(VAS),activity of daily living scale(ADL) score before and after treatment and the clinical efficacy in the two groups were observed.Results:After treatment,the TCM syndrome score and VAS score in the treatment group were lower than those in the control group,while the FMA score and ADL score were higher than those in the control group,and all the differences were statistically significant(P<0.05);The effective rate of the treatment group was 97.50%,and that of the control group was 82.50%.The effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Chinese medicinal plaster for scraping as a medium combined with rehabilitation therapy can effectively relieve shoulder pain,improve clinical symptoms and improve the ability of daily living of patients with shoulder-hand syndrome after stroke.
