3.0TMRI增强以及扩散加权成像对子宫内膜癌侵犯肌层的诊断价值

The value of 3.0T MRI enhancement and diffusion weighted imaging in the diagnosis of muscular invasion ndometrialcarc oma

目的探讨3.0TMRI增强以及扩散加权成像对子宫内膜癌侵犯肌层的诊断价值。方法选取2016年4月-2021年3月收治的子宫内膜癌确诊病例61例,均接受3.0 T常规MRI平扫,以及MRI增强和扩散加权成像扫描检查。根据术后病理检查结果,对不同成像方法的子宫内膜癌肌层侵犯结果进行比较和评价。结果61例子宫内膜癌中,未侵犯肌层16例,浅肌层浸润39例,深肌层浸润6例。根据FIGO(2018年)分期标准,Ⅰ_(A)期10例,Ⅰ_(B)期4例,Ⅱ期37例,Ⅲ期10例。常规MRI平扫显示,61例患者中18例未侵犯肌层,37例为浅肌层浸润,6例为深肌层浸润。MRI增强扫描显示,16例未侵犯肌层,38例为浅肌层浸润,7例为深肌层浸润。DWI扫描显示,14例未侵犯肌层,41例为浅肌层浸润,6例为深肌层浸润。DWI扫描诊断子宫内膜癌是否浸润肌层的敏感度高于常规MRI平扫。结论MRI增强扫描对子宫内膜癌侵犯肌层的诊断价值显著高于常规MRI平扫,而DWI扫描的敏感性较常规MRI平扫更高。

Objective To investigate the diagnostic value of 3.OT MRI enhancement and diffusion weighted imaging in the myometrial invasion of endometrial carcinoma.Methods 61 patients with endometrial cancer admitted from April 2016 to March 2021 were selected.All of them received 3.0T MRI plain scan,MRI enhancement and diffusion weighted imaging.According to the gold standard results of postoperative pathological examination,the results of muscular invasion of endometrial carcinoma with different imaging methods were compared and evaluated.Results Among the 61 cases of endometrial carcinoma,the depth of muscular infiltration showed no muscular invasion in 16 cases,superficial muscular invasion in 39 cases and deep muscular invasion in 6 cases.According to the FIGO(2018)staging criteria,there were 10 patients with stage I_(A),4 patients with stage I_(B),37 patients with stage Ⅱ,and 10 patients with stage Ⅲ.Conventional MRI showed no muscular invasion in 18 of the 61 patients,superficial muscular invasion in 37 cases,and deep muscular invasion in 6 cases.MRI enhanced scan showed no muscular invasion in 16 cases,superficial muscular invasion in 38 cases,and deep muscular invasion in 7 cases.DWI scan showed no muscular invasion in 14 cases,superficial muscular invasion in 41 cases,and deep muscular invasion in 6 cases.The sensitivity of DWI scanning for endometrial carcinoma invasion was higher than that of conventional MRI plain scan and MRI enhancement.Conclusion The diagnostic value of contrast-enhanced MRI scan in endometrial carcinoma is significantly higher than that of conventional MRI scan.The sensitivity of DWI scan was higher than the previous two.
