中药热敷联合乳房按摩在剖宫产产妇中的应用效果

Effects of applying traditional Chinese medicine hot compress combined with manual massage in puerperas after cesarean section

目的探讨中药热敷联合乳房按摩在剖宫产产妇中的应用效果。方法选取2020年1月-2021年10月于笔者所在医院行剖宫产的90例产妇为研究对象,采用随机数字表法将其分为手法按摩组、中药热敷组以及联合应用组3组,每组各30例。手法按摩组予以手法按摩乳房及相应穴位,中药热敷组予以中药热敷乳房,联合应用组予以手法按摩及中药热敷护理。比较3组泌乳始动时间、术后1周乳房肿痛情况、乳房相关并发症及新生儿喂养情况。结果3组泌乳始动时间、乳房肿痛评分比较,差异均有统计学意义(F=12.457,P<0.001;F=15.942,P<0.001);3组乳房相关并发症及新生儿纯母乳喂养情况比较,差异均有统计学意义(χ^(2)=6.537,P=0.038;χ^(2)=8.251,P=0.016)。组间两两比较,联合应用组泌乳始动时间早于手法按摩组及中药热敷组(P<0.05),乳房胀痛评分及乳房相关并发症发生率均低于手法按摩组、中药热敷组(P<0.05),新生儿纯母乳喂养率高于手法按摩组、中药热敷组(P<0.05)。结论剖宫产产妇术后应用中药热敷联合乳房按摩可促进早期泌乳,减轻乳房肿痛,提高新生儿纯母乳喂养率,降低乳房相关并发症的发生率。

Objective To explore the effect of hot compress of traditional Chinese medicine combined with manual massage in puerperas after cesarean section.Methods Totally 90 parturients undergoing cesarean section between January 2020 and October 2021 in the author′s hospital were selected as the research objects.They were divided into a manual massage group,a traditional Chinese medicine hot compress group and a combination group,each of 30,according to a random number table.The manual massage group received manual massage and acupoint nursing,the Chinese medicine hot compress group received Chinese medicine hot compress,while the combination group was given both of the above treatment.The starting time of lactation,breast tenderness at 1 week after operation,neonatal feeding composition and breast related complications were compared among the three groups.Results There were significant differences in the starting time of lactation,breast pain,breast related complications and neonatal feeding among the 3 groups(F=12.457,P<0.001;F=15.942,P<0.001;χ^(2)=6.537,P=0.038;χ^(2)=8.251,P=0.016).The starting time of lactation of the combination group was significantly earlier than that of the other 2 groups(P<0.05),while the breast pain score and the incidence of breast-related complications of the former group was significantly lower than the latter(P<0.05),and the pure breast feeding rate of the former group were significantly higher than that of the other 2 groups(P<0.05).Conclusion The application of Chinese medicine hot compress combined with massage nursing can promote lactation and the rate of exclusive breastfeeding,relieve breast swelling and pain,and lower the incidence of breast complications.
