MHR联合NLR检测与急性ST段抬高型心肌梗死患者PCI术后短期不良预后的相关性 被引量:6

Correlation between MHR combined with NLR and short⁃term prognosis of acute ST⁃segment elevation myocardial infarction patients after PCI

摘要 目的:分析单核细胞/高密度脂蛋白胆固醇比值(MHR)与中性粒细胞/淋巴细胞比值(NLR)对急性ST段抬高型心肌梗死(STEMI)患者短期不良预后的关系。方法:回顾性分析2015年1月至2021年5月在武汉大学中南医院心内科住院并接受急诊经皮冠状动脉介入治疗(PCI)的STEMI患者323例。根据住院期间主要不良心血管事件(MACE)的发生,分为非MACE组(263例)和MACE组(60例)。收集患者的临床资料和实验室检查结果,根据血常规和血生化检查结果计算MHR和NLR值。结果:MACE组的MHR、NLR、收缩压、心率、白细胞水平、甘油三酯、冠脉Gensini评分明显高于非MACE组(P<0.05)。多因素Logistic回归分析表明,MHR和NLR是STEMI患者院内MACE的独立预测因子(P<0.05)。ROC曲线分析提示,MHR预测MACE发生的曲线下面积(AUC)为0.75,其敏感性为76%,特异性为66%。NLR预测MACE发生的AUC为0.72,敏感度为61%,特异性为71%。MHR和NLR联合预测MACE发生的AUC为0.86,敏感度为85%,特异性为73%。结论:MHR和NLR水平是STEMI患者院内MACE发生的独立危险因素,且MHR和NLR联合检测优于单独检测对预测院内MACE的发生。 Objective:To analyze the correlation between monocyte to high density lipoprotein cholesterol(MHR)plus neutrophil/lymphocyte ratio(NLR)and short⁃term adverse prognosis in patients with acute ST⁃segment elevation myocardial infarction(STEMI).Methods:A retrospective analysis of 323 STEMI patients who were hospitalized in the Department of Cardiology,Zhongnan Hospital of Wuhan University and received emergency percutaneous coronary intervention(PCI)from January 2015 to May 2021.According to the occurrence of MACE during hospitalization,they were divided into non⁃MACE group(263 cases)and MACE group(60 cases).The patients'clinical data and labora⁃tory test results were collected,and MHR and NLR values were calculated based on blood routine and blood biochemical test results.Results:The MACE group's MHR,NLR,systolic blood pres⁃sure,heart rate,white blood cell counts,triglycerides,and D⁃dimer levels were significantly higher than those of the non⁃MACE group,and the difference was statistically significant(P<0.05).Logis⁃tic regression analysis showed that MHR and NLR are independent predictors of MACE in STEMI patients(P<0.05).ROC curve analysis indicated that the area of the curve for MHR predicting the occurrence of MACE is 0.75,the sensitivity is 76%,and the specificity is 66%.The area of the curve that NLR predicts the occurrence of MACE is 0.72,the sensitivity is 61%,and the specificity is 71%.The combined area of MHR and NLR predicting the occurrence of MACE is 0.86,the sensi⁃tivity is 85%,and the specificity is 73%.Conclusion:The levels of MHR and NLR are independent risk factors for the occurrence of MACE in STEMI patients,and the combined detection of MHR and NLR is better than the single detection to predict the occurrence of MACE in the hospital.
