温脾养胃活血通络方联合穴位贴敷在脾虚血瘀型消化性溃疡伴Hp感染中的应用效果分析

Analysis of Clinical Efficacy of Wenpi Yangwei Huoxue Tongluo Decoction Combined with Acupoint Application in Treating Peptic Ulcer of Spleen Deficiency and Blood Stasis Type Complicated with Hp Infection

目的探讨温脾养胃活血通络方联合穴位贴敷在脾虚血瘀型消化性溃疡伴Hp感染中的应用效果。方法选取2017年3月至2020年3月遂平县人民医院收治的84例脾虚血瘀型消化性溃疡伴Hp感染患者作为研究对象,按照不同治疗方法将其分为观察组(42例)和对照组(42例),对照组患者采用奥美拉唑三联疗法治疗,观察组患者在对照组治疗的基础上联合应用温脾养胃活血通络方与穴位贴敷,对比观察两组患者临床疗效、Hp转阴率以及胃酸分泌水平。结果治疗结束后,观察组患者总有效率为95.24%、Hp转阴率为88.10%,明显高于对照组患者的总有效率80.95%、Hp转阴率69.05%(χ^(2)=4.086、4.525,P=0.043、0.033);停药4周后,观察组患者基础胃酸分泌量(BAO)、最大胃酸分泌量(MAO)、高峰胃酸分泌量(PAO)均明显低于对照组(t=3.406、5.572、3.347,P均<0.001)。结论在奥美拉唑三联疗法治疗的基础上联合应用温脾养胃活血通络方与穴位贴敷,可有效抑制脾虚血瘀型消化性溃疡伴Hp感染患者的胃酸分泌,改善临床症状,提高Hp转阴率,疗效确切。

Objective To investigate the clinical efficacy of Wenpi Yangwei Huoxue Tongluo(Warming the spleen,nourishing the stomach,promoting blood circulation and dredging collaterals)Decoction combined with acupoint application in treating peptic ulcer of spleen deficiency and blood stasis type complicated with Hp infection.Methods 84 patients with peptic ulcer of spleen deficiency and blood stasis type and Hp infection,admitted to Suiping People’s Hospital from March 2017 to March 2020,were enrolled as research subjects and divided,based on the treatments they received,into study group(n=42)and control group(n=42).Patients in control group were treated with the Omeprazole triple therapy,while patients in study group were treated with Wenpi Yangwei Huoxue Tongluo Decoction combined with acupoint application in addition to the Omeprazole triple therapy.The clinical efficacy,Hp negative conversion rate and gastric acid secretion were compared between the two groups.Results After treatment,the total effective rate and Hp negative conversion rate of patients were respectively 95.24%and 88.10%in study group,obviously higher than the corresponding 80.95%and 69.05%in control group(χ^(2)=4.086 and 4.525,P=0.043 and 0.033);4 weeks after the drug was suspended,the basic acid output(BAO),maximum acid output(MAO)and peak acid output(PAO)of patients in study group were all significantly lower than that in control group(t=3.406,5.572 and 3.347,all P<0.001).Conclusion The combined application of Wenpi Yangwei Huoxue Tongluo Decoction and acupoint application on the basis of the Omeprazole triple therapy can effectively inhibit the gastric acid secretion of patients with peptic ulcer of spleen deficiency and blood stasis type and Hp infection,improve their clinical symptoms,and increase the Hp negative conversion rate.
