消极情绪调节与积极应对在青少年抑郁患者儿童期创伤和自杀意念间的中介作用

The relationship between childhood trauma and suicidal ideation in adolescents with depression: The mediator of negative cognitive emotion regulation and positive coping style

目的 探讨消极认知情绪调节和积极应对方式在青少年抑郁症患者儿童期创伤和自杀意念之间的并行中介作用。方法 选取277例青少年抑郁症患者完成儿童期创伤问卷(CTQ-SF)、认知情绪调节问卷(CERQ)、简易应对方式量表(SCSQ)和自杀意念自评量表(SIOSS)。结果 自杀意念在性别、目前生活环境满意程度、关系亲密的伙伴个数方面比较差异均有统计学意义(P<0.05)。青少年抑郁症患者SIOSS与CTQ-SF、CERQ消极认知情绪调节呈正相关(P<0.01),与SCSQ积极应对方式呈负相关(P<0.01);CTQ-SF与CERQ消极认知情绪调节呈正相关(P<0.01),与SCSQ积极应对呈负相关(P<0.01)。中介模型分析表明,青少年抑郁症患者儿童期创伤对自杀意念有直接效应(P<0.001),消极认知情绪调节、积极应对方式在青少年抑郁症患者儿童期创伤与自杀意念间的单独中介作用显著(P<0.001),中介效应分别占总效应的23%、15%。结论 消极认知情绪调节和积极应对方式在青少年抑郁症患者的儿童期创伤和自杀意念之间存在并行中介作用。

Objective To explore the relationship between childhood trauma and suicidal ideation in adolescents with depression, and the mediating effect of negative cognitive emotion regulation and positive coping style on the relationship between them. Methods A total of 277 adolescents with depression were selected and assessed with Childhood Trauma Scale(CTQ-SF), Cognitive Emotion Regulation Questionnaire(CERQ), Simplified Coping Style Questionnaire(SCSQ) and Self-Rating Idea of Suicide Scale(SIOSS). Results There existed significant differences in SIOSS scores among adolescents with different genders, satisfaction degrees of current living environment and numbers of intimate partners(P<0.05). The SIOSS score was positively correlated with CTQ-SF score and negative cognitive emotion regulation of CERQ(P<0.01), and negatively correlated with positive coping style of SCSQ(P<0.01);CTQ-SF score was positively correlated with negative cognitive emotion regulation of CERQ(P<0.01), and negatively correlated with positive coping style of SCSQ(P<0.01). Mediation model showed that the direct effect of childhood trauma on suicidal ideation was significant(P<0.001);negative cognitive emotion regulation and positive coping style played significant roles in the independent mediation(P<0.001), accounting for 23%, 15% of the total effects respectively. Conclusion Negative cognitive emotion regulation and positive coping style play mediating roles between childhood trauma and suicidal ideation in adolescents with depression.
