补肾活骨胶囊联合体外冲击波治疗髋关节骨髓水肿综合征随访观察

Follow up observation of Bushen huogu capsule combined with extracorporeal shock wave in the treatment of bone marrow edema syndrome

目的观察补肾活骨胶囊联合体外放射式冲击波治疗肾虚血瘀型髋关节骨髓水肿综合征(BMES)的治疗效果。方法以45例肾虚血瘀型BMES患者为观察对象,并按随机数字表法将其分为补肾活骨胶囊组、冲击波组和联合组。补肾活骨胶囊组给予补肾活骨胶囊口服3个月,每次5粒,每日3次;冲击波组采用体外冲击波治疗患侧髋关节,每周2次,持续3个月;联合治疗组即采用口服补肾活骨胶囊及体外冲击波联合治疗3个月。观察治疗前后及治疗后6个月患者基本情况、患髋疼痛程度、关节活动度、Harris髋关节评分及髋关节MRI。结果3组患者治疗后Harris评分均优于治疗前,并且联合治疗组优于补肾活骨胶囊组及冲击波治疗组,差异有统计学意义(P<0.05)。结论补肾活骨胶囊及体外放射式冲击波治疗均能有效治疗肾虚血瘀型BMES,临床效果满意,二者联合应用疗效更佳。

Objective To evaluate the therapeutic effect of Bushen huogu capsule combined with extracorporeal radiation shock wave in the treatment of bone marrow edema syndrome(BMES).Methods From June 2020 to December 2020,45 cases of BMES were selected in Weifang Hospital of traditional Chinese medicine as the observation objects,and they were randomly divided into Bushen huogu capsule treatment group,shock wave treatment group and combined treatment group.The Bushen huogu capsule group was taken Bushen huogu capsule orally for three months,five capsules each time,three times a day;The shock wave group was treated with extracorporeal shock wave twice a week for 3 months;The combined treatment group was treated with Bushen huogu capsule and extracorporeal shock wave for three months.The basic situation,hip pain degree,joint range of motion,Harris hip scores and hip MRI were observed before and after treatment and 6 months after treatment.Results Harris score of three groups after treatment was better than that before treatment,and the combined treatment group was better than the Bushen huogu capsule group and the shock wave treatment group,the difference was statistically significant(P<0.05).Conclusion Bushen huogu capsule and extracorporeal radiation shock wave therapy can effectively prevent and treat BMES and the clinical effect is satisfactory.The combined application of two therapies is more efficacious.
