UPCR与慢性肾脏病进展的相关性研究

摘要 目的:探讨随机尿蛋白与肌酐浓度比(urine protein to vreatinine ratio,UPCR)与慢性肾脏病(chronical kidney disease,CKD)进展的相关性。方法:选取来自公开的“DATADRYAD”数据库(www.datadryad.org)的一组数据,这是日本的一项慢性肾脏疾病治疗和流行病学结局研究,共纳入1050例患者,随访时间40个月。按UPCR值分为3组,Q 1(UPCR≤0.20),Q 2(0.20<UPCR≤2.5),Q 3(UPCR>2.5)。统计该组研究中患者的性别、年龄、体质量指数(BMI)、UPCR、估算的肾小球滤过率(glomerular filtration rate,GFR)、血红蛋白(Hb)、血清白蛋白(ALB)、CKD进展的中位时间和eGFR等指标,探讨UPCR与CKD进展的相关性。结果:在纳入的1050例患者中,平均年龄(67.3±13.6)岁,中位随访时间为26个月。3组间eGFR、CKD进展的中位时间、血红蛋白、白蛋白均有显著差异(P<0.001),且随着UPCR的升高而下降。3组间蛋白尿阳性占比、糖尿病占比、高血压发病率均有显著差异(P<0.001),且随着UPCR的升高而升高。eGFR与UPCR有相关性(rs=0.459,P<0.001)。3组间CKD进展趋势有显著差异(P<0.001),UPCR组别越高,CKD的进展的中位时间越短,差异有统计学意义(P<0.001)。高UPCR组别的(Q 3)患者,UPCR每升高1个单位,CKD进展的风险比UPCR正常组高出7.9倍,有统计学意义(P<0.001)。结论:UPCR是慢性肾脏病进展加重的独立危险因素之一,有望成为判定CKD进展的可靠指标。
