国考背景下临床科室药品费用控制成效的探讨

Discussion of Drug Cost Control Effect in Clinical Departments Under the Background of National Examination

目的了解“国考”以来建湖县人民医院临床科室药品费用控制情况,旨在为进一步减轻住院患者负担,不断完善药品费用深度优化机制。方法于该院HIS中调取2018年~2021年住院科室次均药品费用、药占比、抗肿瘤药使用金额,回顾性评价各项指标的运营及增幅,阐述考核、点评、限停与集采等管控措施作用。结果住院次均药品费用总体水平,由2018年到2021年下降了300元,呈现负增幅的考核单元由2018年15个增加到2021年22个;住院部药占比总体水平,近4年来下降了5.31个百分点。其中,住院肿瘤药使用金额占住院药品总额比例,由2018年16.08%下降到2021年12.80%。结论综合管控临床科室药品费用取得初步成效,系列考评干预机制具有一定利用参考价值,坚定持续改进仍将促使药品费用的再优化。

OBJECTIVE To understand the drug cost control situation in the clinical department of Jianhu County People′s Hospital since the“National Examination”aims to further reduce the burden of inpatients and constantly improve the mechanism for further optimization of drug costs.METHODS From the HIS,the average drug cost,drug proportion and amount of anticancer drugs from 2018 to 2021,retrospectively evaluated the operation and growth of various indicators,and explained the role of control measures such as assessment,comment,restriction and collective acquisition.RESULTS The overall level of drug cost per hospitalization decreased by 300 yuan from 2018 to 2021,and the number of assessment units with negative growth increased from 15 in 2018 to 22 in 2021;the overall proportion of drugs in the inpatient department decreased by 5.31 percentage points in the past four years.Among them,the proportion of inpatient tumor drugs used in the total amount of hospitalized drugs decreased from 16.08%in 2018 to 12.80%in 2021.CONCLUSION Comprehensive control has achieved initial results in clinical department drug costs,and the series of evaluation and intervention mechanism has certain reference value.Firm and continuous improvement will still promote the re-optimization of drug costs.
