某妇幼保健院儿童内分泌专科门诊处方分析

Analysis of Prescriptions of Pediatric Endocrinology Clinic in a Maternal and Child Health Care Hospital

目的分析某妇幼保健院儿童内分泌专科的处方,调查其中主要病种的用药情况,评价其合理性。方法查找2021年7月至9月1763张该专科门诊处方,统计诊断、药物种类,对使用频次最高的药物重点分析。结果抽查的处方中矮小症的治疗药物包括重组人生长激素、维生素D等;性早熟的治疗药物包括曲普瑞林、维生素D等;甲减的主要治疗药物为左甲状腺素钠;肥胖症主要治疗药物为二甲双胍;甲亢的主要治疗药物为甲巯咪唑等。曲普瑞林和生长激素处方剂量过大的比例为2.83%。结论某妇幼保健院儿童内分泌科治疗甲亢、甲减的药物选用合理。性早熟和矮小症的治疗药物选用大部分符合指南推荐,但重组人生长激素不推荐常规使用于性早熟;曲普瑞林和来曲唑不建议常规使用于矮小症。肥胖症治疗选用二甲双胍不合理。少数曲普瑞林和生长激素处方存在剂量不适宜的问题。

OBJECTIVE To analyze the prescriptions of children′s endocrinology specialty in a maternal and child health hospital,investigate the medication of main diseases,and evaluate the rationality of the prescriptions.METHODS 1763 outpatient prescriptions of children′s endocrine specialty from July to September 2021 were found through the hospital′s HIS system.Diagnosis and drug types were counted,and the most frequently used drugs were analyzed.RESULTS Medications for the treatment of short stature included recombinant human growth hormone,vitamin D,etc.Medications for precocious puberty include triptorelin,vitamin D,etc.The main therapeutic drug for hypothyroidism is levothyroxine sodium.The main therapeutic drug for obesity is metformin.The main therapeutic drug for hyperthyroidism are methimazole,etc.The proportion of over-prescribed doses of triptorelin and growth hormone was 2.83%.CONCLUSION Medications for hyperthyroidism and hypothyroidism is reasonable.Most of the therapeutic drugs for precocious puberty and short stature are in line with the guideline recommendations,but recombinant human growth hormone is not recommended for routine use in precocious puberty;triptorelin and letrozole are not recommended for routine use in short stature.The use of metformin for obesity treatment is unreasonable.Some triptorelin and growth hormone prescriptions have problems with inappropriate doses.
