我院处方前置审核的效果研究

A Study on The Effect of Pre-examination of Prescriptions in One Hospital

目的探讨我院2019年实施处方前置审核后其模式应用的优势与不足,进一步促进用药的合理与安全。方法通过前置处方审核系统,制定审方规则,利用点评功能、门诊HIS系统、PASS系统、EXCEL,对2019~2021三年处方进行统计及回顾性分析。结果2019年1月审方系统上线后,处方合理率由2019年的97.33%增长为2021年的98.21%;警示亮灯次数:黑灯基本无变化,红灯降低了14.55%,橙灯提高了29.05%;其中给药剂量、超多日用量、儿童用药不合理率下降,儿童用药不合理率降幅最明显;适应证、规范性审查、重复用药的不合理率连续上升为前三位。结论门诊处方前置审核系统有效提高了门诊处方质量,提高药师审方能力,促进合理用药与我院用药体系建设完善。

OBJECTIVE To explore the advantages and disadvantages of the model application after the implementation of pre-prescription review in our hospital in 2019,and to further promote the rationality and safety of medication.METHODS The pre-prescription review system was used to formulate review rules,and the review function,outpatient HIS system,PASS system,and EXCEL were used to conduct statistical and retrospective analysis of the three-year prescriptions from 2019 to 2021.RESULTS After the trial system was launched in January 2019,the rationality rate of prescriptions increased from 97.33%in 2019 to 98.21%in 2021;the number of warning lights:the black light basically remained unchanged,the red light decreased by 14.55%,and the orange light increased 29.05%;among them,the dosage,excessive daily dosage,and the irrational rate of children′s medication decreased,and the irrational rate of children′s medication decreased the most;the irrational rate of indications,normative review,and repeated medication rose to the top three consecutively.CONCLUSION The pre-examination system of outpatient prescriptions can effectively improve the quality of outpatient prescriptions,improve the ability of pharmacists to examine prescriptions,and promote the construction of rational drug use and drug system in our hospital.
