基于FOCUS-PDCA循环在降低妇科门诊不合理处方中的应用

Application of FOCUS-PDCA Cycle in Reducing Irrational Prescriptions in Gynecological Outpatient Clinics

目的探讨FOUCS-PDCA管理模式在减少不合理处方中的效果。方法选取我院2019年1月~6月妇科不合理处方共229403张,依据FOUCS-PDCA的管理模式,通过流程图,柏拉图,雷达图等工具发现问题、分析原因、解决问题,对比改进前后的成功,再对改进后仍然存在的问题进入下一个FOUCS-PDCA循环,更深度的解决问题,降低不合理处方的干预率。结果①我院妇科不合理处方主要因为4个方面:人员层面;系统层面;环境层面;制度层面。②第一次改进后,妇科不合格处方干预率由5.32%降至4.20%,降幅1.12%;第二次改进后,因诊断不全或不符的处方干预率由1.42%降至0.55%,降幅0.87%。结论FOCUS-PDCA在降低我院妇科不合理处方干预率中效果显著。

OBJECTIVE To explore the effect of FOUCS-PDCA management mode in reducing irrational prescriptions.METHODS A total of 229,403 unreasonable prescriptions for gynecology in our hospital from January to June 2019 were selected.According to the management mode of FOUCS-PDCA,problems were found,analyzed,and solved by tools such as flowcharts,Plato,and radar charts,and compared before and after improvement.After the improvement is successful,the problems that still exist after the improvement will enter the next FOUCS-PDCA cycle to solve the problem more deeply and reduce the intervention rate of unreasonable prescriptions.RESULTS①The unreasonable prescription of gynecology in our hospital is mainly due to four aspects:personnel level;system level;environment level;system level.②After the first improvement,the intervention rate of unqualified prescriptions in gynecology decreased from 5.32%to 4.20%,a decrease of 1.12%;after the second improvement,the intervention rate of prescriptions due to incomplete or inconsistent diagnosis decreased from 1.42%to 0.55%,a decrease of 0.87%.CONCLUSION FOCUS-PDCA has a significant effect in reducing the rate of irrational prescription intervention in gynecology in our hospital.
