PDCA循环在干预围手术期抗菌药物给药时机不适宜中的应用

Effect Evaluation of PDCA Cycle in the Intervention of Inappropriate Timing of Prophylactic Application of Antibiotics during Perioperative Period

目的评价PDCA循环应用于干预围手术期抗菌药物给药时机不适宜的效果。方法抽取PDCA循环干预前(2020年1~6月)、第一轮PDCA循环干预中(2020年7~12月)、第一轮PDCA循环干预后(2021年1~3月)、第二轮PDCA循环干预中(2021年4~6月)、第二轮PDCA循环干预后(2021年7~12月)有应用抗菌药物出院病历每月50份进行点评,对比围手术期抗菌药物给药时机不适宜情况。结果第二轮PDCA循环后,围手术期给药时机不适宜病例比例由干预前的12.67%下降到3.67%,其中切皮前>1 h给药病例比例由干预前的5.67%下降到0.67%。结论PDCA循环能有效促进围手术期抗菌药物给药时机合理,值得推广。

OBJECTIVE To evaluate the effects of PDCA cycle in the intervention of inappropriate timing of antibiotics during perioperative period.METHODS There were 50 discharge medical records of antibiotics per month from January 2020 to December 2021 for review,and the inappropriate timing of antibiotics administration during perioperative period was compared between before PDCA cycle(Jan.-Jun.,2020),during the first PDCA cycle(Jul.-Dec.,2020),after the first PDCA cycle(Jan.-Mar.,2021),during the second PDCA cycle(April-Jun.,2021),after the second PDCA cycle(Jul.-Dec.,2021).RESULTS After the second PDCA cycle,the proportion of patients with unsuitable timing of perioperative administration decreased from 12.67%before the intervention to 3.67%,and the proportion of patients who were administered more than 1 hour before skin excision decreased from 5.67%before the intervention to 0.67%.CONCLUSION PDCA cycle can effectively promote the administration of perioperative antibiotics at a reasonable time,which is worth popularizing.
