血必净注射液对脓毒性休克患者凝血功能障碍的Meta分析

Meta-analysis on the Coagulation Function of Xuebijing Injection in the Treatment of Sepsis Shock

目的:探讨血必净注射液对脓毒性休克患者凝血功能障碍的疗效。方法:通过检索CNKI、WF、VIP、CBM、FMRS及PubMed等论文库,结合手工检索的文献,选取以血必净注射液作为干预措施防治脓毒性休克的临床对照实验,通过查阅全文、评估其质量并获得相应数据,使用Review Manager 5.4软件(Cochrane协作网提供),对其进行Meta分析。经过系统分析,9项研究被纳入,共有791例患者。结果:试验组在增加血小板计数[MD=50.79,95%CI(42.41,59.17),P<0.00001]、缩短凝血酶原时间[MD=-2.02,95%CI(-2.62,-1.43),P<0.00001]、缩短活化部分凝血活酶时间[MD=-9.49,95%CI(-12.05,-6.94),P<0.00001],降低D-二聚体水平[MD=-4.22,95%CI(-4.25,-4.19),P<0.00001]上均优于对照组,差异均具有统计学意义。结论:血必净注射液在脓毒性休克的治疗中可以改善患者的凝血功能障碍。由于研究纳入RCTs数目较少且研究方法学的质量一般,因此还需进一步进行高质量的研究加以论证。

Objective To conduct a systematic evaluation and analysis on the coagulation disorder of Xuebijing Injection in the treatment of Sepsis Shock.Methods Search CNKI,WF,VIP,CBM,FMRS,PubMed and other paper libraries,combined with manual retrieval of literature,for clinical controlled studies of Xuebijing Injection in the treatment of Sepsis Shock.By reviewing the full text,evaluating its quality,and obtaining relevant data,Meta-analysis was performed using Review Manager 5.4 software which is provided by the Cochrane Collaboration.After systematic analysis,a total of 9 RCTs involving 791 patients were included.Results Meta-analysis showed that the experimental group was superior to the control group in increasing platelet count(PLT)[MD=50.79,95%CI(42.41,59.17),P<0.00001],shortening prothrombin time(PT)[MD=-2.02,95%CI(-2.62,-1.43),P<0.00001],shortening activated partial thrombin time(APTT)[MD=-9.49,95%CI(-12.05,-6.94),P<0.00001],and reducing D-dimer(D-D)level[MD=-4.22,95%CI(-4.25,-4.19),P<0.00001],and the differences were statistically significant.Conclusion Xuebijing Injection in the treatment of Sepsis Shock can improve the patient s coagulation disorder.However,due to the small sample size and quality restriction of the literatures,further high-quality studies are needed to demonstrate this.
