中国专家共识制订的进展研究——基于2010~2020年文献的系统评价

A research on the development of expert consensus in China: 2010~2020

目的 了解国内专家共识的发展现状,为其使用者及制订者提供参考。方法 由2名研究者系统检索中国期刊全文数据库(CNKI)和万方数据库(WanFang Database),检索时间为2010年1月1日至2020年12月31日,收集国内近11年自主制订并发布的专家共识。本研究组成员根据专家共识的一般特征结合指南质量评价工具AGREE-Ⅱ的内容进行充分讨论提取本研究关注的方法学质量条目,形成了资料提取表,从专家共识的一般特征和方法学质量的部分内容反映其近11年的发展情况,使用M-K趋势检验法检验专家共识的方法学质量随时间变化的趋势。结果 共纳入1527部临床专家共识,发表量整体呈上升趋势,其部分方法学质量随着时间的推移呈逐年改善的趋势(行充分的文献检索、推荐意见基于系统评价、形成推荐意见考虑必要因素、利益冲突的调查与报道),(P<0.05)。结论 专家共识应按照指南的方法学要求进一步提升质量,以便帮助决策者指导临床实践。

Objective The aim of this study was to systematically review the status and trends of Chinese expert consensus and provide reference for its users and makers. Methods CNKI and WanFang Database were searched by two reviewers from 1 January 2010 to 31 December 2020 for collecting Chinese expert consensus over the past eleven years. According to the content of Appraisal of Guidelines quality Evaluation tool AGREE-Ⅱ, the research group fully discussed and extracted the items of concern in this study to form the data extraction table for describing the general and methodological characteristics of Chinese expert consensus. The M-K test was adopted to identify monotonically increasing or decreasing trends of methodological characteristics over the timespan.Results The total number of expert consensus included in this study was 1527, with an upward in the past eleven years. The methodology quality of some items improved year by year over time, such as systematic literature searching, recommendations based on evidence of systematic reviews, the necessary factors being considered in the formulation of each recommendation, the conflicts of interest investigated and declared(P<0.05). Conclusion The expert consensus should further improve the methodological quality in accordance with the standards of the clinical practice guidelines to help users guide clinical practice.
