全科医生公共卫生核心能力体系构建

Establishment of the public health core competence system for general practitioners

[目的]构建基层全科医生公共卫生核心能力的指标体系,为基层全科医学人才队伍培养评价标准提供参考依据。[方法]对相关文献进行分析总结,形成全科医生公共卫生核心能力体系的初始条目,采用Delphi法选取25名专家进行2轮咨询,应用熵值法确立指标权重。[结果]2轮专家咨询的问卷回收率分别为83.33%和100%;2轮专家咨询的权威系数均大于0.8,分别为0.870和0.894;2轮专家咨询的肯德尔和谐系数为0.252~0.498,且第2轮大于第1轮(P<0.05),1~3级条目的变异系数均小于25%,为14.6%~24.2%;各级指标权重在1.476%~27.522%。构建的全科医生公共卫生核心能力体系包括一级指标5个、二级指标12个、三级指标25个。[结论]本研究所构建的全科医生公共卫生核心能力体系突出全科与公共卫生2大专业特点,可用于全科医生公共卫生核心能力质量评价,使基层复合型人才培养的质量评价标准变得客观、具体、可量化。

Objective The index system of public health core competence for community-level general practitioners is established to provide reference of the cultivation and evaluation standards of community-level general practitioners.Methods The relevant literature was analyzed and summarized to form the initial items of the public health core competence system for general practitioners. Twenty-five experts were selected for two rounds of consultation by Delphi method and entropy method was used to establish the index weight.Results The questionnaire recovery rates in two rounds expert consultation were 83.33% and 100% respectively. The authority coefficients in two rounds expert consultation were both greater than 0.8,which were 0.870 and 0.894 respectively. The Kendall’s concordance coefficient in two rounds expert consultation was between 0.252 and 0.498,and the coefficient in the second round was greater than that in the first round(P<0.05). The coefficient of variation in items of grade 1~3 was less than 25%,all between 14.6% and 24.2%. The index weight at all levels was between 1.476% and 27.522%. The established public health core competence system for general practitioners included five items at level 1,12 items at level 2,and 25 items at level 3.Conclusions Two characteristics which are general practice and public health are highlighted in the research of the public health core competence system for general practitioners. This research can be used for the quality evaluation of the public health core competence for general practitioners to make the cultivation and evaluation standards of community-level interdisciplinary talents objective, specific and quantifiable.
