Study on the quality of health services in aged patients eight weeks after hospitalization and discharge

目的 研究老年患者住院及出院后8周卫生健康服务质量。方法 研究对象为2020年10月至2022年10月在山东医学高等专科学校附属医院(临沂市老年病医院)住院治疗的患者120名。本研究从现象学的角度探索老年患者的主观体验,并进行了半结构化的个人访谈。120名患者(年龄75~100岁)均住院期间接受1次访谈,出院后接受了2次访谈。访谈记录按照不同的主题进行了分析。结果 访谈结果显示,患者发现他们在住院期间往往面对复杂的卫生服务。出院后的患者试图在日常生活中恢复安全感,平衡自己的内外部需求。访谈中出现了五个方面的主要问题:住院期间卫生服务体验、沟通和协调不力、出院后的生活、与近亲的关系以及出院后卫生服务质量。结论 本研究结果表明患者在住院期间参与治疗的机会很少、治疗方案信息匮乏、护理协调欠佳让他们感觉身处困境,他们在日常生活中努力寻找内外需求的平衡点。医务工作者必须做进一步的工作,以确保在没有经济负担的情况下提供以老年人需求和权利为中心的综合卫生服务。

Objective To study the health service quality of aged patients eight weeks after hospitalization and discharge.Methods The subjects were 120 hospitalized patients in Linyi Geriatric Hospital, Affiliated Hospital of Shandong Medical College, from October 2020 to October 2022.This study explored the subjective experience of elderly patients from a phenomenological perspective and conducted semi-structured personal interviews.One hundred and twenty patients(ranging in age from 75 to 100 years)were interviewed once during hospitalization and twice after discharge.The interview records were analyzed according to different topics. Results Interviews revealed that patients often faced complex health services during their stay in the hospital.After leaving the hospital, patients try to regain a sense of security in their daily lives and balance their internal and external needs.Five main areas of concern emerged from the interviews: the experience of health services during hospital stay, poor communication and coordination, life after discharge, relationships with close relatives, and the quality of health services after discharge.Conclusion Results of this study suggest that patients feel trapped during their hospital stay with limited access to treatment, lack of information on treatment options, poor coordination of care, and struggle to find a balance between internal and external needs in their daily lives.Further work must be done to ensure that integrated health services centered on the needs and rights of older persons are provided without financial burdens.
