心脾同治法干预老年慢性心力衰竭临床观察

Efficacy observation of method for simultaneous treatment of heart and spleen on chronic heart failure of elderly patients

目的:观察心脾同治法干预心脾两虚型老年慢性心力衰竭患者的临床疗效。方法:选择老年慢性心力衰竭心脾两虚证患者60例,采用分层随机法分为两组各30例。对照组予西药常规抗心衰治疗,治疗组在对照组基础上加用归脾汤治疗,疗程为4周。观察治疗前后两组血清N端脑钠肽前体(NT-pro BNP)水平、中医证候积分及营养风险指数变化情况。结果:疗程结束后,两组NT-proBNP水平、中医证候积分均较前下降,治疗组优于对照组(P<0.05);治疗组营养指数疗效优于对照组,差异有统计学意义(P<0.05);结论:在常规西药治疗基础上,联合心脾同治法可以显著降低老年慢性心力衰竭心脾两虚证患者中医证候积分、NT-pro BNP水平,提高营养风险指数,且无不良反应。

Objective:To observe the clinical efficacy of method for simultaneous treatment of heart and spleen on chronic heart failure of elderly patients with syndrome of deficiency of heart and spleen.Methods:60 cases of selected elderly patients with chronic heart failure with syndrome of deficiency of heart and spleen were divided into two groups by stratified random method,30 cases in each group.The control group was treated with conventional western medicine for anti-heart failure,while the treatment group was treated plus with Guipi decoction on the basis of control group,and the course of treatment was 4 weeks.The changes of serum NT-proBNP level,TCM syndrome score and nutritional risk index were observed in two groups before and after treatment.Results:After the end of course of treatment,in both groups,NT-proBNP level and TCM syndrome score were lower than before treatment,and the treatment group was better than control group(P<0.05);the curative efficacy of nutritional index was increased,and the treatment group was better than control group,and the difference was statistically significant(P<0.05).Conclusion:On the basis of treatment with conventional western medicine,the combined method for simultaneous treatment of heart and spleen can significantly reduce TCM syndrome score and NT-proBNP level of elderly patients with chronic heart failure with syndrome of deficiency of heart and spleen,improve the nutritional risk index,and has no adverse reactions.
