二甲双胍、达格列净联合生酮饮食对初诊断的肥胖2型糖尿病患者的影响

Effect of metformin and dagaglizin combined with ketogenic diet on newly diagnosed obese patients with type 2 diabetes

目的:观察二甲双胍、达格列净联合生酮饮食模式干预对初诊断的肥胖2型糖尿病患者的影响。方法:随机将60例初诊断的肥胖2型糖尿病患者分为生酮饮食组和常规糖尿病饮食控制组各30例。两组均应用二甲双胍和达格列净,两种饮食模式各干预12周,观察干预前、后血糖、血脂、体重、胰岛素抵抗、尿酸等各指标的变化。结果:干预后,两组患者的体重、体重指数(BMI)、腰围(Waist)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FBG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)均较干预前下降,差异均有统计学意义(P<0.05),生酮饮食组下降更显著;但生酮饮食组尿酸(UA)呈上升趋势,差异有统计学意义(P<0.05);常规糖尿病饮食控制组UA与干预前比较差异无统计学意义(P>0.05)。结论:二甲双胍、达格列净联合生酮饮食对初诊断的肥胖2型糖尿病患者既可以控制体重,又可以控制血糖、血脂。

Objective To observe the effect of metformin and dagaglizin combined with ketogenic dietary pattern on newly diagnosed obese patients with type 2 diabetes.Method Sixty newly diagnosed obese patients with type 2 diabetes were randomly divided into two groups: ketogenic diet group and conventional diabetic diet group, with 30 patients in each group. Both groups were treated with metformin and dagliazin, and the two dietary patterns were treated for 12 weeks each, the changes and significance of blood glucose, blood lipids, body weight, insulin resistance, uric acid and other indicators before and after intervention were observed.Results After intervention, the body weight, body mass index(BMI), waist circumference(Waist), triacylglycerol(TG), total cholesterol(TC), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), fasting blood glucose(FBG), fasting insulin(FINS), and glycosylated hemoglobin(HbA1c) of patients in both groups decreased compared with those before intervention(P<0.05), and the decrease was more significant in the ketogenic diet group;However, UA in ketogenic diet group was on the rise(P<0.05), while that in the conventional diabetic diet group was not significantly changed(P>0.05).Conclusion Metformin and dagaglizin combined with ketogenic diet can control body weight, blood glucose and blood lipids in obese type 2 diabetic patients initially diagnosed.
