光声光谱在细菌性肺炎患者病原菌鉴定中的应用价值

摘要 目的:应用光声光谱分析技术对细菌性肺炎患者及健康人的呼出气进行检测分析,以期探索可用以鉴别细菌性肺炎的特异性气体波峰,探寻细菌性肺炎的早期病原学诊断方法。方法:选择住院的痰细菌培养阳性的肺炎患者12例(肺炎组)及医院健康工作人员6例(健康对照组)为研究对象,用Tedlar采样袋收集其呼出气。用PASTA-1001光声光谱分析仪对上述气体进行检测分析。结果:细菌性肺炎患者呼出气光声光谱检测结果显示,大肠杆菌肺炎患者在1120~1126波数段(E峰)(灵敏度50%、特异度100%)、肺炎克雷伯杆菌肺炎患者在1240~1242波数段(K峰)(灵敏度50%、特异度100%)出现区别于健康对照组的波峰。结论:临床上可根据大肠杆菌肺炎、肺炎克雷伯杆菌肺炎患者呼出气光声光谱检测中的E峰、K峰将其与健康对照者相区分。 Objective:To detect and analyze the exhaled air of patients with bacterial pneumonia and healthy people by photoacoustic spectroscopy,in order to explore the specific gas crest that can be used to distinguish bacterial pneumonia,and explore the early etiological diagnosis method of bacterial pneumonia.Methods:12 hospitalized pneumonia patients(pneumonia group)and 6 hospital health workers(healthy control group)were selected as subjects,and their exhaled air was collected with Tedlar sampling bag.The above gases were detected and analyzed by PASTA-1001 photoacoustic spectrometer.Results:The photoacoustic spectroscopy of exhaled air of patients with bacterial pneumonia showed that E.coli pneumonia patients in 1120~1126 wave number segment(E peak)(sensitivity 50%,specificity 100%),Klebsiella pneumoniae pneumonia patients in 1240~1242 wave number segment(K peak)(sensitivity 50%,specificity 100%),different from the healthy control group.Conclusion:Patients with E.coli pneumonia and Klebsiella pneumoniae pneumonia can be distinguished from healthy controls according to the photoacoustic spectra of E peak and K peak in exhaled air.
