肌内效贴联合超声引导下注射治疗对脑卒中偏瘫肩痛患者疼痛及上肢功能康复的影响

Effects of kinesio tape combined with ultrasound-guided injection therapy on pain and upper limb functional rehabilitation in stroke patients with hemiplegic shoulder pain

目的探讨肌内效贴(KT)联合超声引导下注射治疗对脑卒中偏瘫肩痛(HSP)患者疼痛及上肢功能康复的影响。方法选取2017年12月至2019年6月赣州市人民医院康复科收治的90例脑卒中HSP患者作为研究对象,按照随机数字表法分为对照组(45例)与观察组(45例)。对照组采用常规康复治疗及超声引导下肩关节注射治疗,观察组在对照组基础上采用KT治疗,共治疗4周,比较两组患者肩痛程度、上肢功能及肩关节被动活动度。结果治疗前,两组患者的视觉模拟评分法(VAS)比较,差异无统计学意义(P>0.05);治疗后24 h、4周,两组患者的VAS评分均低于本组治疗前,治疗后4周,两组患者的VAS评分均低于本组治疗后24 h,差异有统计学意义(P<0.05);观察组治疗后24 h、4周的VAS评分均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者的Fulg-Meyer运动功能评估法(FMA)评分及肩关节被动外旋、外展、前屈活动度比较,差异无统计学意义(P>0.05);治疗后,观察组的FMA评分、肩关节被动外旋、外展、前屈活动度高于对照组,差异有统计学意义(P<0.05)。结论在脑卒中HSP患者中采用KT联合超声引导下注射治疗效果确切,在改善上肢运动功能、肩痛症状及肩关节被动活动度方面效果更佳,有利于提升患者康复效果。

Objective To investigate the effect of kinesio tape(KT)combined with ultrasound-guided injection therapy on pain and functional rehabilitation of upper limbs in patients with hemiplegic shoulder pain(HSP)after stroke.Methods A total of 90 HSP patients with stroke admitted to the Department of Rehabilitation of Ganzhou People's Hospital from December 2017 to June 2019 were selected as the research subjects,and they were divided into the control group(45 cases)and the observation group(45 cases)according to the random number table method.The control group received conventional rehabilitation therapy and ultrasound-guided shoulder joint injection therapy,while the observation group received KT therapy on the basis of the control group for a total of 4 weeks of treatment.The degree of shoulder pain,upper limb function and passive range of motion of the shoulder joint were compared between the two groups.Results There was no significant difference in the visual analogue scale(VAS)score between the two groups before treatment(P>0.05).The VAS scores of the observation group at 24 h and 4 weeks after treatment were lower than those of the control group,and the differences were statistically significant(P<0.05).There were no significant differences in Fugl-Meyer assessment(FMA)score and the passive external rotation,abduction and forward flexion range of motion of the shoulder joint between the two groups before treatment(P>0.05).After treatment,the FMA score,the passive external rotation,abduction and forward flexion of the shoulder joint of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion KT combined with ultrasound-guided injection is effective in HSP patients with stroke,and it is more effective in improving upper limb motor function,shoulder pain symptoms and passive range of motion of shoulder joint,which is beneficial to improve the recovery effect of patients.
