临床药师在硝基咪唑类抗菌药物处方点评中的应用效果

Application effect of clinical pharmacists in prescription review of Nitroimidazole antibiotics

目的探讨临床药师在硝基咪唑类抗菌药物处方点评中的应用效果。方法选取2020年10月至2021年12月滕州市中医医院3000份临床硝基咪唑类抗菌药物处方为研究对象,按照处方开具时间的先后顺序分为常规组(2020年10月至2021年5月)和干预组(2021年6月至12月),各1500份。其中常规组处方在使用期间未给予临床药师干预;而干预组处方在使用期间均给予临床药师干预。比较两组不合理处方发生情况、硝基咪唑类抗菌药物的用药频率(DDDs)和用药强度(AUD)。结果干预组适应证不合理、药物选择不合理、预防用药不合理、剂型和给药途径不合理、联合用药不合理、重复用药、用法用量不合理、处方超量未注明原因、处方无诊断、缺损签名或盖章和其他不合理处方类型占比均低于常规组,差异有统计学意义(P<0.05)。干预组硝基咪唑类抗菌药物的DDDs和AUD均低于常规组,差异有统计学意义(P<0.05)。结论经药师干预的专项处方点评可以有效降低硝基咪唑类抗菌药物的用药频率和用药强度,提高该类药物临床应用的合理性,为保障患者用药安全奠定良好的基础。

Objective To explore the application effect of clinical pharmacists in prescription review of Nitroimidazoles.Methods A total of 3000 clinical prescriptions of Nitroimidazoles from Tengzhou Hospital of Traditional Chinese Medicine from October 2020 to December 2021 were selected as the study subjects,and they were divided into the conventional group(October 2020 to May 2021)and the intervention group(June to December 2021)according to the order of prescription issue,with 1500 prescriptions each.In the conventional group,no clinical pharmacist intervention was given during the use of the prescription.In the intervention group,all prescriptions were given clinical pharmacists intervention during the use period.The occurrence of irrational prescription,defined daily doses(DDDs)and antibiotics use density(AUD)of Nitroimidazoles were compared between the two groups.Results After intervention,the proportion of unreasonable indications,unreasonable drug selection,unreasonable preventive medication,unreasonable dosage form and route of administration,unreasonable combination medication,repeated medication,unreasonable usage and dosage,unexplained reasons for excessive prescription,no diagnosis of prescription,defective signature or seal and other unreasonable prescription types were lower than those before intervention,the differences were statistically significant(P<0.05).DDDs and AUD of Nitroimidazole antibiotics after intervention were lower than those before intervention,the differences were statistically significant(P<0.05).Conclusion the special prescription comments intervened by pharmacists can effectively reduce the medication frequency and intensity of Nitroimidazole antibiotics,improve the rationality of their clinical application,and lay a good foundation for ensuring the medication safety of patients.
