麻杏石甘汤加减治疗慢性阻塞性肺疾病急性加重期的临床效果

Clinical effect of Maxing Shigan decoction on acute exacerbation of chronic obstructive pulmonary disease

目的观察麻杏石甘汤加减治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床效果。方法选取2019年1月—2021年1月江苏省常州市金坛区中医医院收治的AECOPD患者60例,依照随机分组对照原则分为观察组和对照组,各30例。对照组予常规治疗,观察组在对照组基础上加用麻杏石甘汤加减治疗,2组均以7 d为1个疗程,共治疗2个疗程。比较2组治疗效果,治疗前后炎性因子[白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]水平、中医证候评分变化。结果观察组患者治疗总有效率为100.00%,高于对照组的86.67%(χ^(2)=4.286,P=0.038);治疗2个疗程后,2组IL-8、TNF-α、CRP水平及咳嗽、喘息、气短评分均较治疗前下降,且观察组低于对照组(P<0.05或P<0.01)。结论麻杏石甘汤加减治疗AECOPD的临床效果肯定,能显著改善炎性反应,缓解咳嗽、喘息等临床症状,值得临床推广应用。

Objective To observe the clinical effect of Maxing Shigan decoction in treating acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods From January 2019 to January 2021,60 patients with AECOPD were selected from Changzhou Jintan District Hospital of Traditional Chinese Medicine.According to the principle of random grouping and control,they were divided into observation group and control group,with 30 cases in each group.The control group was given routine treatment,while the observation group was given modified Maxing Shigan decoction on the basis of the control group.Both groups took 7 days as a course of treatment,a total of 2 courses of treatment.The therapeutic effect,the changes of inflammatory factors(IL-8,TNF-α,CRP)level and TCM syndrome score before and after treatment were compared between the two groups.Results The total effective rate of patients in the observation group was 100.00%,higher than 86.67%in the control group(χ^(2)=4.286,P=0.038).After two courses of treatment,the levels of IL-8,TNF-α,CRP and the scores of cough,wheezing and shortness of breath in the two groups were lower than those before treatment,and the scores in the observation group were lower than those in the control group(P<0.05 or P<0.01).Conclusion Maxing Shigan decoction has a positive clinical effect in treating AECOPD.It can significantly improve inflammatory reaction,relieve cough,wheezing and other clinical symptoms,and is worthy of clinical application.
