胃苓汤合五皮饮治疗原发性肾病综合征水肿期(水湿浸渍)患者的疗效观察

Therapeutic effect of the Weiling decoction plus Wupi Yin on primary nephrotic syndrome in edema stage(water immersion) patients

目的:研究胃苓汤合五皮饮治疗原发性肾病综合征(Primary Nephrotic Syndrome,PNS)水肿期(水湿浸渍)患者的临床疗效及安全性。方法:选择广宁县人民医院于2018年4月-2020年4月收治的PNS水肿期(水湿浸渍)患者60例,随机分成两组(对照组和观察组),各30例。两组基础治疗均给予饮食控制和入量控制,对照组另外口服西医常规药物治疗,观察组以对照组方案为基础另加胃苓汤合五皮饮治疗,总疗程8周,记录两组的临床疗效,以及治疗前后的24小时尿蛋白定量(24-hour Urinary Protein,24 h Upro)、血清尿素氮(Blood Urea Nitrogen,BUN)、血清肌酐(Serum Creatinine,Scr)、血清白蛋白(Albumin, Alb)、血清总胆固醇(Total Cholesterol,TC)、血清三酰甘油(Triglyceride,TG)及不良反应数据,最后对数据进行统计分析。结果:8周疗程完成后,在总有效率方面,观察组(93.3%,28/30)显著比对照组(70.0%,21/30)高,差异具有统计学意义(P<0.05),两组患者的24 h Upro、Alb、BUN、Scr、TC、TG水平均较同组疗程开始前好转(P<0.05),且观察组24 h Upro、Alb、BUN、TC、TG的改善程度优于同期对照组(P<0.05),差异具有统计学意义。两组不良反应率比较,差异无统计学意义(P>0.05)。结论:胃苓汤合五皮饮治疗PNS水肿期(水湿浸渍)患者临床疗效满意,不增加不良反应。

Objective:To study the clinical efficacy and safety of the Weiling decoction(胃苓汤)plus Wupi Yin(五皮饮)on primary nephrotic syndrome in edema stage(water immersion)patients.Methods:Sixty patients with PNS edema(water immersion)treated in Guangning People’s Hospital from April 2018 to April 2020 were randomly divided into two groups(the control group and the observation group),with 30 cases in each group.The basic treatment in the two groups were diet control and intake control.The control group was added with oral conventional Western medicine.The observation group was treated with the Weiling decoction plus Wupi Yin based on the scheme in the control group.The total course of treatment was 8 weeks.The clinical efficacy,24-hour urinary protein quantification(24 h Upro),serum urea nitrogen(BUN),serum creatinine(Scr),serum albumin(Alb),serum total cholesterol(TC),serum triglyceride(TG)and adverse reaction data before and after the course of treatment were recorded.Finally,the data were statistically analyzed.Results:After 8 weeks of treatment,the total effective rate in the observation group(93.3%,28/30)was significantly higher than that in the control group(70.0%,21/30),and the difference was statistically significant(P<0.05).The levels of 24 h Upro,Alb,BUN,Scr,TC and TG in the two groups were ameliorated than those before the treatment(P<0.05),the amelioration of 24 h Upro,Alb,BUN,TC and TG in the observation group was better than that in the control group(P<0.05),and the differences were statistically significant.There was no significant difference in adverse reaction rate between the two groups(P>0.05).Conclusion:The clinical effect of the Weiling decoction plus Wupi Yin is satisfactory in the treatment of patients with PNS edema stage(water immersion),and it does not increase the incidence of adverse reactions.
