复方黄柏液简易负压冲洗技术在糖尿病足溃疡伤口处理中的应用

Application of compound Huangbai solution simple negative pressure irrigation technique in the treatment of diabetic foot ulcer wound

目的:探讨复方黄柏液简易负压冲洗技术在糖尿病足溃疡伤口处理中的应用效果及伤口的改善情况,同时为复方黄柏液简易负压冲洗技术的临床运用和推广提供科学依据。方法:本研究选择2021年3月-2022年2月贵州某三甲中医医院收治的符合糖尿病足溃疡诊疗标准的住院患者64例,根据入院顺序使用随机数字表法分成对照组和观察组,每组患者各32例,对所有招募入组患者予常规治疗基础上按照各组换药方案进行换药治疗,对照组给予0.9%氯化钠溶液冲洗联合简易负压治疗,观察组给予复方黄柏液冲洗联合简易负压治疗,每日冲洗1次,3~7 d更换一次负压材料,总干预时间为21 d。以两组患者的伤口体积缩小率、压疮愈合计数(Pressure Ulcer Scale for Healing,PUSH)及伤口愈合率作为评价指标,采用SPSS 22.0软件进行数据录入和统计分析。结果:两组患者完成预期治疗,两组患者治疗前基线资料差异无统计学意义,患者的基线资料具有可比性。治疗后观察组创面面积缩小、创面愈合率、PUSH评分均优于对照组,差异具有统计学意义(P<0.05)。结论:复方黄柏液简易负压冲洗技术在糖尿病足溃疡伤口处理中能够促进创面面积缩小,减少创面渗液量,提高创面愈合效果,具有经济、安全、操作简便等优点,可在临床推广运用。

Objective:To investigate the application effect of compound Huangbai solution(复方黄柏液)simple negative pressure irrigation technique in the treatment of diabetic foot ulcer wound,and to provide scientific basis for the clinical application and promotion of compound Huangbai solution simple negative pressure irrigation technique.Methods:In this study,64 inpatients admitted to a third-class TCM Hospital in Guizhou from March 2021 to February 2022 were selected and divided into the control group and the observation group by random number table method according to the admission order.There were 32 patients in each group.On the basis of conventional treatment,all the enrolled patients were given dressing change treatment according to the dressing change plan in each group.The control group was given normal saline irrigation plus simple negative pressure treatment,and the observation group was given compound Huangbai solution irrigation plus simple negative pressure treatment,once a day,and the negative pressure material was replaced once 3-7 days,with a total intervention time of 21 days.The wound area reduction rate,PUSH sore and wound healing rate in the two groups were used as evaluation indexes,and SPSS 22.0 software was used for data entry and statistical analysis.Results:The two groups of patients completed the expected treatment,and there was no statistical difference in the baseline data before treatment between the two groups,and the baseline data of the patients were comparable.After treatment,the wound area reduction rate,the wound healing rate,PUSH score in the observation group were superior to those in the control group,the differences were statistically significant(P<0.05).Conclusion:Compound Huangbai solution simple negative pressure irrigation technique can promote the reduction of wound volume,reduce the amount of wound seepage and improve the wound healing effect in the treatment of diabetic foot ulcer wound.It has the advantages of being economical,safety,simple operation and so on,and can be popularized and applied in clinic.
