原发性肝癌腹腔镜肝切除术后患者癌症复发恐惧状况分析

Influencing factors of fear of cancer recurrence in primary hepatocellular carcinoma patients underwent laparoscopic hepatectomy

目的分析原发性肝癌(HCC)腹腔镜肝切除术后患者的癌症复发恐惧状况。方法前瞻选取2018年1月至2021年2月接受腹腔镜肝切除术的HCC患者96例,患者均于术后1 d接受癌症复发恐惧量表(FCRI)评估,并设计基线资料填写表,记录患者相关基线资料情况,比较不同资料特征患者的FCRI评分,分析可能影响HCC腹腔镜肝切除术后患者癌症复发恐惧状况的相关因素。结果96例HCC腹腔镜肝切除术后患者存在不同程度癌症复发恐惧水平,平均FCRI得分(97.42±10.69)分;初中及以下学历、社会支持低、家庭月收入<3000元、C级的HCC腹腔镜肝切除术后患者FCRI评分高于中专或高中学历、大专及以上学历、中高等社会支持、家庭月收入≥3000元、A级和B级的HCC腹腔镜肝切除术后患者,差异均有统计学意义(P<0.05);其他不同特征HCC腹腔镜肝切除术后患者FCRI评分比较,差异无统计学意义(P>0.05);多元线性回归分析结果显示,初中及以下学历、社会支持低、家庭月收入<3000元、C级均是导致HCC腹腔镜肝切除术后患者癌症复发恐惧高的影响因素(P<0.05)。结论初中及以下学历、社会支持低、家庭月收入<3000元、C级均是HCC腹腔镜肝切除术后患者癌症复发恐惧高的影响因素。

Objective To analyze the fear for cancer recurrence(FCR) in primary hepatocellular carcinoma(HCC) patients underwent laparoscopic hepatectomy(LH).Methods Totally 96 HCC patients underwent LH from January 2018 to February 2021 were prospectively enrolled. On day 1 of operation, the fear of cancer recurrence inventory(FCRI) was performed to assess. A baseline data table was designed to evaluate the FCRI scores of patients with different characteristics, and the FCR-related factors were analyzed.Results Among these patients, the different levels of FCR were noted, with average FCRI score of(97.42±10.69)points. FCRI scores of HCC patients receiving junior high school education or below, low social support, family monthly income <3,000 Yuan, Grade C of postoperative score were significantly higher than those receiving special secondary or senior high school education, college education or above, high social support, family monthly income ≥3,000 Yuan, Grade A and Grade B of postoperative score(P<0.05). Difference in other FCRI scores of patients with different characteristics was not statistically significant(P>0.05). Multiple linear regression analysis showed that junior high school education or below, low social support, family monthly income <3,000 Yuan and Grade C were higher factors for FCR(P<0.05).Conclusion For HCC patients underwent LH,Junior high school education or below, low social support, family monthly income <3,000 Yuan and Grade C of postoperative score are higher factors for FCR.
