门诊糖尿病患者中成药使用情况分析

Usage of Chinese Patent Medicine in outpatient diabetes

目的了解中成药在门诊糖尿病患者中的使用情况及趋势,为临床合理用药提供参考。方法查询医院信息系统,统计2019年1~12月2000张诊断中含有“糖尿病”门诊处方,遵循中医药基本理论及临床治疗学特点,对使用中成药的频率品种、适应证、用药禁忌和用法用量等进行分析点评。结果门诊糖尿病患者中成药的使用率为15.2%(304/2000),理血剂的使用最多(63.8%)。59.9%的处方存在适应证不适宜,未能辨证施药;未发现使用影响肝肾功能的药物禁忌;3.3%的处方存在成分重复的中成药,中成药与西药联合使用可能出现相互作用的处方较多;24.3%的处方存在单次用量小的现象,4.6%的处方存在单次用量大的现象。结论门诊糖尿病患者的中成药处方应加强监管,提高临床合理用药水平。

Objective To explore the usage of Chinese Patent Medicine(CPM) in outpatients diabetes, to provide reference for proper clinical application.Methods During January and December 2019, totally 2,000 outpatient CPM prescriptions containing "diabetes" were collected from the hospital information system. Based on the fundamental theory and characteristics of traditional Chinese medicine(TCM), this paper analyzed and reviewed on the frequency, variety, indications, contraindications, usage and dosage of CPM.Results Among the outpatients diabetic, the proportion of the patients with CPM utilization was 15.2%(304/2,000);the utilization rate of blood-regulating drugs was the most(63.8%);prescriptions with unsuitable indications accounted for 59.9%, and failed to differentiate medicines;no contraindications were found for the use of drugs affecting liver and kidney function;3.3% of the prescriptions had CPM with repeated components, and the combination of CPM and Western medicine had more possible interactions;24.3% of the prescriptions had small single dosage and 4.6% for large single dosage.Conclusion The prescription of CPM for outpatients diabetic should be strengthened and the level of rational drug use should be improved.
