摘要
目的研究鹼性成纖維細胞生長因子(bFGF)在乳腺癌細胞中的表達特徵,尋找有助於診斷乳腺癌的生物學標記。方法收集乳腺增生症、乳腺纖維腺瘤和乳腺癌等的手術切除標本各22例,常規石蠟包埋,連續切片(4μm),分別做HE染色和用ABC法進行bFGF(l:200)、血管標記抗體CD34(l:100)免疫組織化學染色,參考Weidner和Maeda方法進行微血管密度(MVD)測定。結果在乳腺增生症和乳腺纖維腺瘤中的肌上皮細胞、導管上皮細胞、血管内皮細胞、血管壁平滑肌細胞和纖維母細胞等均表達bFGF,其陽性產物主要位於胞漿,肌上皮細胞和導管上皮細胞的陽性產物部分見於胞核,但乳腺癌細胞的陽性產物主要位於胞膜,少數位於胞漿,胞核陰性,與良性增生性病變形成鮮明的對照。22例乳腺癌bFGF膜陽性者17例(占77.3%),其MVD 90.56±23.62。腫瘤直徑≥4cm(10/22)時,MVD增高達93.19±20.75。乳腺增生症MVD 42.05±12.26.乳腺纖維腺瘤MVD 43.14±18.21,與乳腺癌相比有顯著差異(P<0.01)。結論l.bFGF免疫染色陽性產物分佈特徵:在乳腺癌細胞中主要分佈在胞膜,在良性增生性病變細胞中主要位於胞漿和胞核。乳腺癌的MVD明顯高於良性增生性病變。2.利用bFGF免疫染色的分佈特徵結合MVD可預測乳腺良性增生病變的惡性傾向或有助於乳腺癌的組織學診斷。
Objective To study the expression significance of bFGF in benign and malignant breast hyperplasia lesion and looking for a biological mark for diagnosis of the breast cancer.Methods Tissues of 66 cases,in which 22 cases were breast carcinoma,22 cases were fibroadenoma and 22 cases were adenosis,were fixed and embedded by routine pathologic technique and serial sections(4μm).Immunostains of bFGF(1:200)and CD34(l:100)were conducted by ABC method.The MVD was estimated with Weidner and Maeda Method.Results bFGF positive products,in the fibroadenoma and adenosis were mainly in the cytoplasm and nuclei of the cells of myoepithelial,hyperplasia ductal epithelial and endoepithelial of vessel,but mostly(17/22,77.3%)occurring on the membrane of the cells in the breast carcinoma,MVD of which was 90.56±23.62 and greater than MVD of both fibroadenoma(43.14±18.21)and adenosis(42.05±12.26)(p<0.01).The MVD increased when the diameter of carcinoma was over 4cm in diameter.Conclusions The positive products of bFGF immunostain in breast carcinoma was mainly on the carcinoma cell membrane and significantly different from that mostly in the cytoplasm and nuclear of cells of the breast benign hyperplasia lesions.The located features of bFGF expression combined with the MVD may be useful to predict the tendency of malignant transformation of benign hyperplasia lesions or to diagnosis of breast carcinoma.
