摘要
目的探究超声弹性成像(UE)对乳房占位性病变的鉴别诊断价值。方法选取2018年10月-2021年10月南京市江宁中医院收治的82例乳腺结节患者,共104个病灶,依次进行彩色多普勒超声与UE检查,分析UE的诊断价值,以病理学结果为金标准,比较两种检测方式的敏感性、特异性、误诊率、漏诊率与诊断符合率。结果良性病灶的UE评分低于恶性病灶(P<0.05),且91.67%良性病灶的UE评分≤3分,90.63%恶性病灶的UE评分≥4分,差异有统计学意义(P<0.05);UE诊断乳房占位性病变的敏感性、特异性高于彩色多普勒超声,而误诊率、漏诊率低于彩色多普勒超声(P<0.05);UE对乳房占位性病变的诊断符合率(91.35%)高于彩色多普勒超声(86.54%)(P<0.05)。结论超声弹性成像在乳房占位性病变的鉴别诊断中具有重要作用,其对乳腺良恶性病变的诊断敏感性、特异性均较为理想,真实性与可靠性高。
Objective To explore the value of ultrasound elastography(UE)in the differential diagnosis of breast occupying lesions.Methods A total of 82 patients with breast nodules admitted to Jiangning Hospital of Traditional Chinese Medicine in Nanjing from October 2018 to October 2021 were selected,with a total of 104 lesions.Color Doppler ultrasound and UE were performed in turn to analyze the diagnostic value of UE.The pathological results were used as the gold standard to compare the sensitivity,specificity,misdiagnosis rate,missed diagnosis rate and diagnostic coincidence rate of the two detection methods.Results The sensitivity and specificity of UE in the diagnosis of breast occupying lesions were higher than those of color Doppler ultrasound,while the misdiagnosis rate and missed diagnosis rate were lower than those of color Doppler ultrasound(P<0.05).The diagnostic coincidence rate of UE(91.35%)was higher than that of color Doppler ultrasound(86.54%)(P<0.05).Conclusion Ultrasound elastography plays an important role in the differential diagnosis of breast occupying lesions.Its diagnostic sensitivity and specificity for benign and malignant breast lesions are ideal,and its authenticity and reliability are high.
