双柏散外敷合四君子汤加减对胃癌化疗患者消化道不良反应、生活质量及静脉炎发生率的影响

Influence of External Application of Shuangbai Powder Combined with Modified Sijunzi Decoction on Gastrointestinal Adverse Reactions,Quality of Life and Incidence of Phlebitis in Patients with Gastric Cancer Undergoing Chemotherapy

目的:探讨双柏散外敷合四君子汤加减对胃癌化疗患者消化道不良反应、生活质量及静脉炎发生率的影响。方法:选取2019年3月-2022年3月东台市中医院收治的胃癌化疗患者82例,按照随机数字表法分两组,对照组予以常规化疗,观察组在对照组基础上予以双柏散外敷合四君子汤加减治疗。评估临床疗效、中医症候积分及生活质量,并记录不良反应及静脉炎发生情况。结果:治疗3个月后,观察组临床总有效率60.98%高于对照组的41.46%,差异有统计学意义(P<0.05);观察组胃脘胀痛、食少纳差、倦怠乏力、大便异常及恶心呕吐等中医症候积分均明显低于对照组,差异均有统计学意义(P<0.05);观察组胃肠道反应、静脉炎发生率均比对照组低,差异均有统计学意义(P<0.05);两组肝、肾功能损伤及周围神经毒性发生率比较差异均无统计学意义(P>0.05)。治疗3个月后,观察组胃癌患者生活质量问卷(QLQ-STO_(2)2)评分明显低于对照组,差异有统计学意义(P<0.05)。结论:双柏散外敷合四君子汤加减用于胃癌化疗效果显著,能够促进症状改善,减轻消化道不良反应,降低静脉炎发生率,同时还能提高生活质量,可供临床参考应用。

Objective:To investigate the effect of external application of Shuangbai Powder combined with modified Sijunzi Decoction on gastrointestinal adverse reactions,quality of life and incidence of phlebitis in patients with gastric cancer undergoing chemotherapy.Method:A total of 82 patients with gastric cancer treated in Dongtai Hospital of Traditional Chinese Medicine from March 2019 to March 2022 were selected and divided into two groups according to the random number table method.The control group received conventional chemotherapy,and the observation group received external application of combined with modified Sijunzi Decoction on the basis of the control group.The clinical efficacy,TCM symptom score and quality of life were evaluated,and adverse reactions and phlebitis were recorded.Result:After 3 months of treatment,the total clinical effective rate of the observation group was 60.98%,higher than 41.46%of the control group,the difference was statistically significant(P<0.05);the TCM syndrome scores such as epigastric pain,lack of appetite,fatigue,abnormal stool,nausea and vomiting in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05);the incidences of gastrointestinal reaction and phlebitis were lower than those of the control group,the differences were statistically significant(P<0.05);there were no significant differences in the incidences of liver and kidney damage and peripheral neurotoxicity between the two groups(P>0.05).After 3 months of treatment,the quality of life questionnaire of stomach 22(QLQ-STO_(2)2)score of the observation group was significantly lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:External application of Shuangbai Powder combined with modified Sijunzi Decoction has significant curative effect on gastric cancer chemotherapy,which can improve symptoms,reduce adverse reactions of digestive tract,reduce the incidence of phlebitis,and improve the quality of life at the same time,which can be used for clinical reference application.
