生物反馈电刺激联合认知行为疗法对产后女性性功能障碍的作用

Effect of Biofeedback Electrical Stimulation Combined with Cognitive Behavioral Therapy on Postpartum Female Sexual Dysfunction

目的:研究生物反馈电刺激与认知行为疗法(CBT)相结合对产后女性性功能障碍(FSD)的治疗效果。方法:将2019年10月-2021年6月在空军军医大学第一附属医院妇产科门诊就诊的118例产后FSD患者按随机数字表法分为对照组和试验组,每组59例。对照组接受生物反馈电刺激治疗,试验组接受生物反馈电刺激和CBT联合治疗。治疗前和治疗12周后,对两组盆底肌肌力、膀胱生物学进行评估,并使用9项患者健康问卷(PHQ-9)和7项广泛性焦虑障碍量表(GAD-7)对抑郁和焦虑进行评估,治疗12周后对患者性生活进行评估。结果:治疗前,两组盆底肌肌力、膀胱初感及最大充盈量、PHQ-9和GAD-7得分比较,差异均无统计学意义(P>0.05);治疗后,试验组以上指标及性生活质量均优于对照组(P<0.05)。结论:生物反馈电刺激结合CBT可有效改善盆底肌功能和膀胱功能,缓解产后FSD患者的抑郁和焦虑,从而提高性生活质量。

Objective:To study the therapeutic effect of biofeedback electrical stimulation combined with cognitive behavioral therapy(CBT)on postpartum female sexual dysfunction(FSD).Method:A total of 118postpartum FSD patients who visited the Department of Obstetrics and Gynecology Outpatient,the First Affiliated Hospital of Air Force Military Medical University from October 2019 to June 2021 were divided into the control group and the experimental group according to the random number table method,with 59 cases in each group.The control group was treated with biofeedback electrical stimulation,and the experimental group was treated with the combination of biofeedback electrical stimulation and CBT.Before and after treatment for 12 weeks,the pelvic floor muscle strength and bladder biology of the two groups were evaluated,and depression and anxiety were evaluated by using patient health questionnaire-9(PHQ-9)and generalized anxiety disorde-7(GAD-7).After 12 weeks of treatment,the sexual life of the patients were evaluated.Result:Before treatment,there were no significant differences in pelvic floor muscle strength,bladder initial sensation maximum filling volume,PHQ-9 and GAD-7 scores between the two groups(P>0.05).After treatment,the above indexes and sexual life quality of the experimental group were better than those of the control group(P<0.05).Conclusion:Biofeedback electrical stimulation combined with CBT can effectively improve the pelvic floor muscle function and bladder function,and alleviate the depression and anxiety of postpartum FSD patients,thereby improving the quality of sexual life.
