H型高血压患者血小板功能变化及临床意义

The changes of platelet function in patients with H-type hypertension and its clinical significance

目的探讨H型高血压患者血小板功能变化及临床意义。方法121例原发性高血压患者,根据血浆同型半胱氨酸(Hcy)水平分为H型高血压组(Hcy≥10μmol/L,64例)和对照组(Hcy<10μmol/L,57例)。检测并比较两组P-选择素、β-血小板球蛋白(β-TG)、平均血小板体积(MPV)、血小板最大聚集率、Hcy水平。结果H型高血压组P-选择素(158.71±6.84)ng/L、β-TG(26.76±3.22)μg/L、Hcy(15.80±4.40)μmol/L均明显高于对照组的(149.47±8.88)ng/L、(23.19±3.15)μg/L、(7.51±2.16)μmol/L,MPV(13.71±1.42)fL、经花生四烯酸(AA)诱导血小板最大聚集率(78.04±11.34)%、经二磷酸腺苷(ADP)诱导血小板最大聚集率(76.39±10.42)%均明显大于对照组的(10.19±1.26)fL、(64.59±9.51)%、(65.56±9.23)%,差异有统计学意义(P<0.05)。结论H型高血压患者血小板被活化,血小板功能发生改变,易发生心脑血管疾病。H型高血压患者应及早检测血小板功能,进行早期预防、干预,以降低心脑血管疾病的发生风险。

Objective To discuss the changes of platelet function in patients with H-type hypertension and its clinical significance.Methods 121 patients with essential hypertension were divided into the H-type hypertension group(Hcy≥10μmol/L,64 cases)and the control group(Hcy<10μmol/L,57 cases)according to plasma homocysteine(Hcy)levels.P-selectin,β-thromboglobulin(β-TG),mean platelet volume(MPV),maximum platelet aggregation rate,and Hcy levels were measured and compared in both groups.Results The P-selectin(158.71±6.84)ng/L,β-TG(26.76±3.22)μg/L,and Hcy(15.80±4.40)μmol/L in the H-type hypertension group were significantly higher than(149.47±8.88)ng/L,(23.19±3.15)μg/L,and(7.51±2.16)μmol/L in the control group;the MPV(13.71±1.42)fL,maximum platelet aggregation induced by arachidonic acid(AA)(78.04±11.34)%,and maximum platelet aggregation induced by adenosine diphosphate(ADP)(76.39±10.42)%in the H-type hypertension group were significantly greater than(10.19±1.26)fL,(64.59±9.51)%,and(65.56±9.23)%in the control group;the differences were statistically significant(P<0.05).Conclusion Platelets are activated and platelet function is altered in patients with H-type hypertension,which predisposes them to cardiovascular and cerebrovascular diseases.Patients with H-type hypertension should have their platelet function tested early for early prevention and intervention to reduce the risk of cardiovascular and cerebrovascular diseases.
