外周血结核感染T细胞斑点试验对活动性结核的诊断价值

Diagnostic Value of T Cell Spot Test in Peripheral Blood for Active Tuberculosis Infection

目的 以外周血结核感染T细胞斑点试验研究其在活动性结核诊断中的效果。方法 将本院检验科50例疑似结核患者分为试验组与对照组,试验组接受外周血结核感染T细胞斑点试验,对照组接受结核干扰素酶免试验。通过对试验组患者与对照组患者的试验研究,收集试验指标指数,对比两组患者的敏感度、特异度、准确度、动脉血压变异率(PPV)、NPV、LR+、LR-指标。结果 试验组即以外周血结核感染T细胞斑点试验患者在检验敏感度上为87.9%,特异度为84.1%,PPV、NPV指标分别为73.8%、94.9%,准确度达84.2%,LR+为(6.10±0.31),LR-为(0.13±0.30);对照组即以结核干扰素酶免试验的患者在检验敏感度上为57.9%,特异度为57.3%,PPV、NPV指标分别为42.5%、74.9%,准确度达59.2%,LR+为(1.59±0.29),LR-为(0.68±2.31)。根据试验组检验指标与对照组检验指标的对比能够发现,试验组检验指标明显由于对照组检验指标,且组间数据差异有统计学意义(P<0.05)。结论 以外周血结核感染T细胞斑点试验在活动性结核诊断中的应用价值较高,其灵敏度、特异度及准确性较高,可为临床诊治提供更充分的依据。

Objective To study the effect of T cells in the diagnosis of active tuberculosis in peripheral blood by dot test. Methods Fifty suspected tuberculosis patients were divided into experimental group and control group. The experimental group received peripheral blood tuberculosis infection T cell spot test, and the control group received tuberculosis interferon enzyme immunity test. Through the experimental study of patients in the experimental group and the control group, the test index index was collected to compare the sensitivity, specificity, accuracy, PPV(rate of arterial blood pressure variation), NPV, LR+, LR-indexes of the two groups of patients. Results In the test group, the sensitivity, specificity, PPV and NPV indexes were 87.9%,84.1%, 73.8% and 94.9%, respectively. The accuracy reached 84.2%, LR+ was(6.10±0.31), LR-was(0.13±0.30). The sensitivity, specificity, PPV and NPV indexes of the control group were 57.9%, 57.3%, 42.5% and 74.9%, respectively. The accuracy reached 59.2%, LR+ was(1.59±0.29), LR-was(0.68±2.31). According to the comparison between the test indexes of the experimental group and the control group, the test indexes of the experimental group were significantly higher than the test indexes of the control group, and the data difference between the two groups was significant, with statistical significance(P<0.05). Results The peripheral blood T cell spot test of tuberculosis infection has high application value in the diagnosis of active tuberculosis, and its sensitivity, specificity and accuracy are high, which can provide more sufficient basis for clinical diagnosis and treatment.
