非瓣膜性心房颤动患者发生心力衰竭的影响因素

Influencing factors of heart failure in patients with non-valvular atrial fibrillation

目的:分析非瓣膜性心房颤动患者发生心力衰竭的影响因素。方法:回顾性分析2018年10月至2020年7月该院收治的70例非瓣膜性心房颤动患者的临床资料,统计非瓣膜性心房颤动患者发生心力衰竭情况,并采用Logistic回归分析非瓣膜性心房颤动患者发生心力衰竭的影响因素。结果:70例非瓣膜性心房颤动患者发生心力衰竭12例,占17.14%,设为发生组;未发生心力衰竭58例,占82.86%,设为未发生组;发生组的性别、年龄、病程、糖尿病占比、高脂血症占比、心房颤动类型与未发生组比较,差异均无统计学意义(P>0.05);发生组体质量指数≥24 kg/m^(2)、心室率>100次/min、合并高血压、合并营养不良、左心房内径>33 mm、红细胞体积分布宽度>16%占比均高于未发生组,差异有统计学意义(P<0.05);Logistic回归分析结果显示,体质量指数≥24 kg/m^(2)、心室率>100次/min、合并高血压、合并营养不良、左心房内径>33 mm、红细胞体积分布宽度>16%均为影响非瓣膜性心房颤动患者发生心力衰竭的危险因素(OR>1,P<0.05)。结论:体质量指数≥24 kg/m^(2)、心室率>100次/min、合并高血压、营养不良、左心房内径>33 mm、红细胞体积分布宽度>16%均为影响非瓣膜性心房颤动患者发生心力衰竭的危险因素。

Objective:To analyze influencing factors of heart failure in patients with non-valvular atrial fibrillation.Methods:The clinical data of 70 patients with non-valvular atrial fibrillation admitted to the hospital from October 2018 to July 2020 were retrospectively analyzed.The non-valvular atrial fibrillation patients with heart failure were set as occurrence group,and those without heart failure were set as non-occurrence group.The proportion of the two groups was counted.Logistic regression analysis was used to analyze the influencing factors of heart failure in the patients with non-valvular atrial fibrillation.Results:There were 12 cases of heart failure in the 70 patients with non-valvular atrial fibrillation,accounting for 17.14%,and 58 cases without heart failure,accounting for 82.86%.There were no significant differences in gender,age,course of disease,diabetes,hyperlipidemia and type of atrial fibrillation between the two groups(P>0.05).The proportions of body mass index≥24 kg/m^(2),ventricular rate>100 beats/min,hypertension,malnutrition,left atrial diameter>33 mm,and red blood cell volume distribution width>16%in the occurrence group were higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that body mass index≥24 kg/m^(2),ventricular rate>100 beats/min,hypertension,malnutrition,left atrial diameter>33 mm,red blood cell volume distribution width>16%were the risk factors for heart failure in the patients with non-valvular atrial fibrillation(OR>1,P<0.05).Conclusions:Body mass index≥24kg/m^(2),ventricular rate>100 beats/min,hypertension,malnutrition,left atrial diameter>33 mm,red blood cell volume distribution width>16%are the risk factors for heart failure in the patients with non-valvular atrial fibrillation.
