摘要
目的:比较经皮椎体后凸成形术(PKP)与经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩性骨折(OVCF)患者的效果。方法:选取2018年11月至2020年10月该院收治的80例OVCF患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各40例。对照组行PVP治疗,观察组行PKP治疗,比较两组术前、术后2个月影像学指标(后凸Cobb角、椎体前缘高度比)水平、功能障碍[Oswestry功能障碍指数(ODI)]评分、疼痛程度[视觉模拟评分法(VAS)]评分,以及并发症发生率。结果:术后2个月,观察组Cobb角小于对照组,椎体前缘高度比高于对照组,差异均有统计学意义(P<0.05);观察组ODI、VAS评分均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为5.00%(2/40),低于对照组的20.00%(8/40),差异有统计学意义(P<0.05)。结论:PKP治疗OVCF患者可改善影像学指标水平,降低功能障碍评分、疼痛程度评分和并发症发生率,效果优于PVP治疗。
Objective:To compare effects of percutaneous kyphoplasty(PKP)and percutaneous vertebroplasty(PVP)in treatment of osteoporotic vertebral compression fracture(OVCF).Methods:A prospective study was conducted on 80 patients with OVCF admitted to the hospital from November 2018 to October 2020.They were divided into control group and observation group according to the random number table method,40cases in each.The control group was treated with PVP,while the observation group was treated with PKP.The imaging index levels(kyphosis Cobb angle,anterior vertebral height ratio),the dysfunction[Oswestry Disability Index(ODI)]score,pain degree[Visual analog Scale(VAS)score],and the incidence of complications compared between the two groups before and 2 months after the surgery.Results:2 months after the surgery,the Cobb angle of the observation group was smaller than that of the control group,the anterior vertebral height ratio was higher than that of the control group,and the differences were statistically significant(P<0.05).The ODI and VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 5.00%(2/40),which was lower than 20.00%(8/40)in the control group,and the difference was statistically significant(P<0.05).Conclusions:PKP treatment the OVCF patients can improve the levels of imaging indexes and reduce the dysfunction score,the pain score and the incidence of complications.Moreover,it is superior to PVP treatment.
