改良Proseal喉罩与改良普通喉罩在无痛纤维支气管镜检查中的应用效果比较

Comparison of application effects of modified Proseal laryngeal mask and modified common laryngeal mask in painless fiberoptic bronchoscopy

目的:比较改良Proseal喉罩与改良普通喉罩在无痛纤维支气管镜检查中的应用效果。方法:选取2019年5月至2020年7月该院收治的80例无痛纤维支气管镜检查患者进行前瞻性研究,按照随机数字表法将其分为观察组与对照组各40例。两组均行无痛纤维支气管镜检查,检查中对照组使用改良普通喉罩,观察组使用改良Proseal喉罩,比较两组不同时间[检查前(T_(1))、插罩即刻(T_(2))、拔罩即刻(T_(3))]收缩压(SBP)、舒张压(DBP)、心率(HR)、平均动脉压(MAP)水平,气道管理指标(首次置罩成功率、置罩用时、气道密封压)水平,并发症发生率,以及治疗满意度。结果:T_(2)、T_(3)时,观察组SBP、DBP、HR和MAP水平均低于对照组,对照组SBP、DBP、HR和MAP水平均高于T_(1)时,差异有统计学意义(P<0.05);观察组首次置罩成功率和气道密封压明显高于对照组,置罩用时短于对照组,差异有统计学意义(P<0.05);观察组拔罩12、24 h喉咙疼痛发生率明显低于对照组,差异有统计学意义(P<0.05);观察组治疗满意度为92.50%,明显高于对照组的72.50%,差异有统计学意义(P<0.05)。结论:改良Proseal喉罩应用于无痛纤维支气管镜检查可提高首次置罩成功率、气道密封压和治疗满意度,缩短置罩用时,以及降低SBP、DBP、HR、MAP和喉咙疼痛发生率,优于改良普通喉罩效果。

Objective:To compare application effects of modified Proseal laryngeal mask and modified common laryngeal mask in painless fiberoptic bronchoscopy.Methods:A prospective study was conducted on 80 patients with painless fiberoptic bronchoscopy admitted to the hospital from May 2019 to July 2020.They were divided into observation group and control group according to the random number table method,40 cases in each.Both groups underwent painless fiberoptic bronchoscopy.During the examination,the control group used the modified ordinary laryngeal mask,while the observation group used the modified Proseal laryngeal mask.The systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR)and mean arterial pressure(MAP)at different time[before examination(T_(1)),immediately after insertion(T_(2)),immediately after removal(T_(3))],the airway management index levels(success rate of first placement,time of placement,airway sealing pressure),the incidence of complications,and the treatment satisfaction were compared between the two groups.Results:At T_(2)and T_(3),the SBP,DBP,HR and MAP levels in the observation group were lower than those in the control group;the SBP,DBP,HR and MAP levels in the control group were higher than those at T_(1);and the differences were statistically significant(P<0.05).The success rate of the first mask placement and the airway sealing pressure in the observation group were significantly higher than those in the control group,the time of mask placement was shorter than that in the control group,and the differences were statistically significant(P<0.05).The incidence of throat pain 12 h and 24 h after mask removal in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the treatment satisfaction of the observation group was 92.50%,which was significantly higher than 72.50%of the control group,and the difference was statistically significant(P<0.05).Conclusions:The modified Proseal laryngeal mask in painless fiberoptic bronchoscopy can improve the success rate of first placement,the airway sealing pressure and the treatment satisfaction,shorten the time of placement,and reduce the SBP,DBP,HR and MAP levels,incidence of throat pain.Moreover,it is superior to modified common laryngeal mask.
