点阵激光联合复方倍他米松注射液治疗斑秃的临床效果分析

Clinical Effect Analysis of Spot Laser Combined With Compound Betamethasone Injection on Alopecia Areata

目的分析点阵激光联合复方倍他米松注射液治疗斑秃的临床效果。方法选取内蒙古科技大学包头医学院第一附属医院皮肤科门诊接收的60例斑秃患者,随机分为参照组和观察组,各30例。参照组患者给予复方倍他米松注射液导入治疗,观察组患者给予点阵激光联合复方倍他米松注射液导入治疗。比较两组治疗前和治疗后的毛发镜检查情况、斑秃面积、生活质量、血液流变学、免疫学指标、甲襞微循环、治疗总有效率、疾病复发率以及不良反应发生率。结果治疗后,观察组患者毛囊密度(89.51±3.82)个/cm^(2)和毛发密度(123.42±3.99)根/cm^(2)高于参照组,毳毛率(10.29±1.67)%低于参照组(P<0.05)。观察组的斑秃面积(2.75±0.27)cm^(2)少于参照组,生活质量评分(3.79±2.82)分低于参照组(P<0.05)。观察组患者全血黏度高切、全血黏度低切水平低于参照组,TNF-α和TGF-β1也低于参照组(P<0.05)。观察组患者模糊管襻、畸形管襻、粒线流发生率均低于参照组(P<0.05)。观察组的治疗总有效率为96.77%,高于参照组的73.33%(P<0.05)。观察组的疾病复发率为3.33%,低于参照组的26.67%(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论斑秃患者给予点阵激光联合复方倍他米松注射液治疗,可优化毛发镜检查情况,维持患者较好生活质量,提高治疗总有效率,且不良反应发生较少。

Objective To analyze the clinical effect of spot laser combined with compound betamethasone injection in the treatment of alopecia areata.Methods A total of 60 patients with alopecia areata who were admitted to department of Dermatology,the First Affiliated Hospital,Baotou Medical College,Inner Mongolia University of Science and Technology from January 2021 to February 2022 were randomly divided into reference and observation groups,30 patients each.Patients in the reference group were given compound betamethasone injection lead-in treatment,patients in the observation group were given dot matrix laser combined with compound betamethasone injection lead-in treatment.Pretreatment and posttreatment hair microscopy,alopecia areata,quality of life,hemorheology,immunological indices,microcirculation of the folds,overall response rate to treatment,disease recurrence rate,and incidence of adverse effects were compared between the two groups.Results After treatment,the hair follicle density(89.51±3.82)one/cm^(2) and hair density(123.42±3.99)roots/cm^(2) in the observation group were higher than those in the reference group,and the hair loss rate(10.29±1.67)%was lower than that in the reference group(P<0.05).The area of alopecia areata(2.75±0.27)cm^(2) and the score of quality of life(3.79±2.82)points in the observation group were lower than those in the reference group(P<0.05).The high shear and low shear levels of whole blood viscosity in the observation group were lower than those in the reference group,TNF-αAnd TGF-β1 was also lower than that of the reference group(P<0.05).The incidence of blurred loop,deformed loop and granulocyte flow in the observation group was lower than that in the reference group(P<0.05).The total effective rate of the observation group was 96.77%higher than that of the reference group(73.33%)(P<0.05),and the disease recurrence rate of the observation group was 3.33%lower than that of the reference group(26.67%)(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of alopecia areata with dot matrix laser combined with compound betamethasone injection can optimize the hair microscopic examination,maintain the better quality of life of patients,improve the total effective rate of treatment,and reduce adverse reactions.
