摘要
目的 研讨两种影像学技术(CT、MRI)在两种颈椎病(脊髓型颈椎病、神经根型颈椎病)中的诊断价值.方法 选取邵阳市中心医院2021年1月-2022年1月收治的50例脊髓型颈椎病、50例神经根型颈椎病患者为研究对象.两组患者均接受CT检查、MRI检查,并对两种检查方法的检出情况、影像征象表现进行分析和比较.结果 在神经根型颈椎病的诊断方面,CT、MRI的诊断准确率差异不明显,无统计学意义(P>0.05).在脊髓型颈椎病的诊断方面,MRI的诊断准确率高于CT,统计学意义明确(P<0.05).在两组类型颈椎病影像征象检出方面,CT有3项征象(椎小关节骨质增生、椎间盘钙化、椎间盘积气)检出率较MRI高,MRI有5项征象(椎间盘突出、黄韧带增厚、硬膜囊受压、脊髓变性、神经根受压)检出率较CT高,统计学意义明确(P<0.05);两种检查方法的椎间孔狭窄、椎管狭窄检出率差异不明显,无统计学意义(P>0.05).结论 对神经根型颈椎病的诊断,CT以及MRI的诊断准确性均较高,而在脊髓型颈椎病诊断方面,MRI优势更为明显,将CT和MRI联合开展诊断,可提升诊断效果.
Objective To compare the utility of two imaging techniques(CT,MRI)in two kinds of cervical spondylosis(cervical spondylosis myelopathy,cervical spondylosis radiculosa).Methods A total of 50 patients with cervical spondylotic myelopathy and 50 patients with cervical spondylotic radiculopathy admitted to our hospital from January 2021 to January 2022 were selected as the research objects.The two groups of patients were examined by CT and MRI,and the detection and imaging manifestations of the two methods were analyzed and compared.Results In the diagnosis of cervical spondylotic radiculopathy,there was no significant difference in the diagnostic accuracy between CT and MRI(P>0.05).In the diagnosis of cervical spondylotic myelopathy,the diagnostic accuracy of MRI was higher than that of CT,with statistical significance(P<0.05).In two groups of type cervical spondylosis imaging signs detection,CT has three signs(vertebra small joint bone hyperplasia,intervertebral disc calcification,intervertebral disc pneumatosis)high detection rate with MRI,the MRI has five signs(thickening of intervertebral disc herniation,yellow ligament and dural sac compression and deformation of the spinal cord and nerve root compression)high detection rate with CT,statistics meaning(P<0.05);there was no significant difference in the detection rates of foraminal stenosis and spinal canal stenosis between the two methods(P>0.05).Conclusion CT and MRI have high diagnostic accuracy in the diagnosis of cervical spondylotic radiculopathy,while MRI has more obvious advantages in the diagnosis of cervical spondylotic myelopathy.The combination of CT and MRI can improve the diagnostic effect.
